Je v dobr\u00e9m fyzick\u00e9m stavu, v minulosti nem\u011bl \u017e\u00e1dn\u00e9 v\u00e1\u017en\u011bj\u0161\u00ed zdravotn\u00ed probl\u00e9my a p\u0159i<br \/>pot\u00e1p\u011bn\u00ed dosud neutrp\u011bl \u017e\u00e1dn\u00e1 zran\u011bn\u00ed. Nebere pravideln\u011b \u017e\u00e1dn\u00e9 l\u00e9ky. Pot\u00e1p\u00ed se p\u011bt let, jeho pr\u016fm\u011brn\u00fd po\u010det ponor\u016f ro\u010dn\u011b je kolem 40. B\u011bhem roku, v kter\u00e9m se p\u0159ihodila n\u00ed\u017ee popsan\u00e1 nehoda, uskute\u010dnil 4 ponory.<br \/>Tento pot\u00e1p\u011b\u010d m\u00e1 pom\u011brn\u011b \u0161irok\u00e9 zku\u0161enosti s r\u016fzn\u00fdmi podm\u00ednkami &ndash; pot\u00e1p\u011bl se ve studen\u00fdch vnitrozemsk\u00fdch vod\u00e1ch i v Karibsk\u00e9m mo\u0159i. P\u0159ed p\u011bti leti m\u011bl diagnostikovan\u00fd &ldquo;nata\u017een\u00fd&rdquo; z\u00e1dov\u00fd sval, co\u017e se \u00fasp\u011b\u0161n\u011b vyl\u00e9\u010dilo a od t\u00e9 doby nem\u011bl \u017e\u00e1dn\u00e9 pot\u00ed\u017ee. Pravideln\u011b cvi\u010d\u00ed bez jak\u00fdchkoliv probl\u00e9m\u016f \u010di omezen\u00ed.<\/p>\n<p style=\"text-align: justify;\"><strong>Ponory<\/strong><br \/>Ponory se uskute\u010dnily v \u0159ece dostate\u010dn\u011b hlubok\u00e9 na to, aby v n\u00ed na dn\u011b v\u011bzel vrak lodi. Teplota vody byla 18 &deg;C a teplota vzduchu mezi 18-19 &deg;C. Viditelnost byla asi na 3 metry a musel plavat proti proudu o s\u00edle p\u0159ibli\u017en\u011b 4 uzl\u016f. Oba ponory prov\u00e1d\u011bl se stla\u010den\u00fdm vzduchem a pou\u017e\u00edval pot\u00e1p\u011b\u010dsk\u00fd po\u010d\u00edta\u010d. Tento po\u010d\u00edta\u010d nab\u00edzel r\u016fzn\u00e9 &ndash; pot\u00e1p\u011b\u010dem voliteln\u00e9 &ndash; algoritmy.<br \/>Jeho p\u0159edchoz\u00ed ponory se uskute\u010dnily v tepl\u00e9m oce\u00e1nsk\u00e9m prost\u0159ed\u00ed a tehdy si zvolil pon\u011bkud m\u00e9n\u011b konzervativn\u00ed algoritmus. Stejn\u00fd algoritmus se rozhodl pou\u017e\u00edt i pro chladn\u011bj\u0161\u00ed ponory v \u0159ece.<br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">Sice nelze s ur\u010ditost\u00ed \u0159\u00edci, zdali volba algoritmu nep\u0159isp\u011bla k n\u00ed\u017ee popsan\u00e9mu pr\u016fb\u011bhu ud\u00e1losti, ale v \u00favahu se tato skute\u010dnost vz\u00edt mus\u00ed. <br \/>Prvn\u00ed ponor za\u010dal v 10:53 dopoledne a byl do maxim\u00e1ln\u00ed hloubky 20 metr\u016f s celkovou dobou pod vodou 41 minut. V\u00edce\u00farov\u0148ov\u00fd profil odpov\u00eddal hloubce potopen\u00e9ho vraku lodi. P\u0159i n\u00e1vratu u\u010dinil t\u0159\u00edminutovou bezpe\u010dnostn\u00ed p\u0159est\u00e1vku v hloubce 5 metr\u016f. B\u011bhem tohoto ponoru nezaznamenal \u017e\u00e1dn\u00e9 pot\u00ed\u017ee.<br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">P\u0159est\u00e1vka na povrchu trvala 1 hodinu 21 minut a pot\u00e1p\u011b\u010d tvrd\u00ed, \u017ee b\u011bhem n\u00ed poci\u0165oval chlad. Tak\u00e9 k tomu dodal, \u017ee si nevzal pot\u0159ebn\u00e9 oble\u010den\u00ed. (Vzhledem k tomu, \u017ee nem\u00e1me \u00fadaje o pot\u00e1p\u011b\u010dov\u011b teplot\u011b a &ndash; v tomto sm\u011bru &ndash; ani \u017e\u00e1dn\u00e9 dal\u0161\u00ed \u00fadaje, nem\u016f\u017eeme tvrdit, ale ani vyvracet, zdali m\u011bl jeho pocit chladu n\u011bjakou souvislost s dal\u0161\u00edmi ud\u00e1lostmi. Uv\u00e1d\u00edme to jen pro \u00faplnost.) Druh\u00fd ponor za\u010dal v 12:56 odpoledne a uskute\u010dnil se do maxim\u00e1ln\u00ed hloubky 22 metr\u016f. Celkov\u00e1 doba pod vodou tentokr\u00e1t \u010dinila 48 minut.<br \/>P\u0159ibli\u017en\u011b po 20 minut\u00e1ch b\u011bhem druh\u00e9ho ponoru pot\u00e1p\u011b\u010d\u016fv kolega necht\u011bn\u011b zavadil o masku &ldquo;na\u0161eho&rdquo; pot\u00e1p\u011b\u010de a uvolnil mu ji. Ten &ndash; kdy\u017e si ji \u010distil, upravoval a znovu nasazoval &ndash; dosti rychle vystoupal o 3 metry v\u00fd\u0161e. (Tuto skute\u010dnost zaznamenal i po\u010d\u00edta\u010d.) P\u0159i zm\u00edn\u011bn\u00e9m incidentu n\u011bkolikr\u00e1t polknul trochu vody. Zv\u00fd\u0161ila se mu frekvence d\u00fdch\u00e1n\u00ed, proto\u017ee za\u017e\u00edval ur\u010dit\u00e9 nap\u011bt\u00ed a stres. Pot\u00e1p\u011b\u010d se pon\u011bkud uklidnil, ur\u010dit\u011b &ldquo;nezpanika\u0159il&rdquo;. <br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">Jakmile si vy\u010distil masku a za\u010dal znovu pravideln\u011b d\u00fdchat, p\u0159ipojil se op\u011bt ke sv\u00e9mu spole\u010dn\u00edkovi (kter\u00fd v\u016fbec nezaznamenal, co se p\u0159ihodilo). <br \/>Pokra\u010doval v ponoru a\u017e do tlaku 68 bar, kdy\u017e jeho po\u010d\u00edta\u010d za\u010dal signalizovat &ldquo;povinnou bezpe\u010dnostn\u00ed zast\u00e1vku&rdquo;. Oba pot\u00e1p\u011b\u010di provedli v\u00fdstup pomalej\u0161\u00edm tempem ne\u017e norm\u00e1ln\u011b, a sice rychlost\u00ed p\u0159ibli\u017en\u011b 3-6 metr\u016f za minutu. V hloubce 5 metr\u016f prodlou\u017eili svou bezpe\u010dnostn\u00ed zast\u00e1vku na 5 minut. Na hladinu se vyno\u0159ili v 1:44 odpoledne.<\/p>\n<p style=\"text-align: justify;\"><strong>P\u0159\u00edznaky<\/strong><br \/>Ve 4 hodiny odpoledne, tedy asi 2 hodiny po posledn\u00edm ponoru, poc\u00edtil brn\u011bn\u00ed v prav\u00e9 noze. Sbalil si celou v\u00fdbavu a v\u00fdstroj a odjel dom\u016f. Po p\u0159\u00edjezdu v\u0161e vybalil, opl\u00e1chnul a ulo\u017eil. Brn\u011bn\u00ed sice nes\u00edlilo, ale za\u010dala jej bolet pata prav\u00e9 nohy. Bolest byla natolik siln\u00e1, \u017ee mu zt\u011b\u017eovala ch\u016fzi.<br \/>Navzdory t\u011bmto p\u0159\u00edznak\u016fm p\u0159ipravil ve\u010de\u0159i. Kdy\u017e p\u0159\u00edznaky neustoupily ani do 8 hodin ve\u010der, za\u010dalo jej to zneklid\u0148ovat. Na nal\u00e9h\u00e1n\u00ed man\u017eelky zavolal 24-hodinovou pot\u00e1p\u011b\u010dskou pohotovostn\u00ed slu\u017ebu DAN. Po d\u016fkladn\u00e9m popisu sv\u00fdch ponor\u016f, pr\u016fvodn\u00edch okolnost\u00ed i aktu\u00e1ln\u00edch p\u0159\u00edznak\u016f mu l\u00e9ka\u0159 DAN doporu\u010dil vyhled\u00e1n\u00ed m\u00edstn\u00ed nemocnice, kter\u00e1 m\u00e1 hyberbarickou komoru i person\u00e1l vy\u0161kolen\u00fd pro diagnostiku a l\u00e9\u010dbu zran\u011bn\u00fdch pot\u00e1p\u011b\u010d\u016f. <br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">Do nemocnice p\u0159ijel asi v 9 hod. ve\u010der. Slu\u017ebu konaj\u00edc\u00ed l\u00e9ka\u0159 se poradil s plicn\u00edm specialistou a s neurologem. Pot\u00e1p\u011b\u010dovy p\u0159\u00edznaky se sice nejevily jako typick\u00e9 pro dekompresn\u00ed nemoc (DCS), ale po p\u0159ekontrolov\u00e1n\u00ed profil\u016f jeho ponor\u016f a vyhodnocen\u00ed v\u00fdvoje p\u0159\u00edznak\u016f stanovil o\u0161et\u0159uj\u00edc\u00ed l\u00e9ka\u0159 jako diagn\u00f3zu dekompresn\u00ed nemoc (DCS) typu II.&nbsp; V\u00fdsledkem bylo nasazen\u00ed hyperbarick\u00e9 kysl\u00edkov\u00e9 terapie podle tabulky \u010d. 6 n\u00e1mo\u0159nictva Spojen\u00fdch st\u00e1t\u016f. Za\u010dalo se s n\u00ed v 1 hodinu r\u00e1no, t.j. 9 hodin po objeven\u00ed se prvn\u00edch p\u0159\u00edznak\u016f a pobyt v hyperbarick\u00e9 komo\u0159e skon\u010dil v 6 hodin r\u00e1no. B\u011bhem zm\u00edn\u011bn\u00e9 terapie podstatn\u011b zesl\u00e1blo brn\u011bn\u00ed v prav\u00e9 noze, ale naopak zes\u00edlila bolestivost prav\u00e9 paty. <br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">O t\u0159i a\u017e \u010dty\u0159i hodiny pozd\u011bji provedl l\u00e9ka\u0159 &ndash; specialista na hyperbarickou medic\u00ednu &ndash; dal\u0161\u00ed vy\u0161et\u0159en\u00ed a konstatoval, \u017ee u pot\u00e1p\u011b\u010de ur\u010dit\u00e9 p\u0159\u00edznaky p\u0159etrv\u00e1vaj\u00ed. Proto rozhodl o dal\u0161\u00ed terapii, tentokr\u00e1t podle tabulky \u010d. 5 n\u00e1mo\u0159nictva Spojen\u00fdch st\u00e1t\u016f. Po jej\u00edm ukon\u010den\u00ed byly p\u0159\u00edznaky jen ob\u010dasn\u00e9 a ve srovn\u00e1n\u00ed s p\u016fvodn\u00edmi zna\u010dn\u011b slab\u0161\u00ed. P\u0159i zobrazen\u00ed p\u00e1te\u0159e nebyla patrn\u00e1 \u017e\u00e1dn\u00e1 druhotn\u00e1 p\u0159\u00ed\u010dina st\u00e1le je\u0161t\u011b p\u0159etrv\u00e1vaj\u00edc\u00edch (i kdy\u017e slab\u0161\u00edch a ob\u010dasn\u00fdch) p\u0159\u00edznak\u016f.<br \/>L\u00e9ka\u0159 doporu\u010dil pot\u00e1p\u011b\u010di, aby minim\u00e1ln\u011b na jeden m\u011bs\u00edc nepl\u00e1noval \u017e\u00e1dn\u00e9 pot\u00e1p\u011bn\u00ed a tak\u00e9, aby alespo\u0148 jeden t\u00fdden nelet\u011bl letadlem. Rovn\u011b\u017e m\u011bl nejm\u00e9n\u011b na jeden t\u00fdden vypustit n\u00e1ro\u010dn\u00e1 t\u011blesn\u00e1 cvi\u010den\u00ed.<\/p>\n<p style=\"text-align: justify;\"><strong>Komplikace<\/strong><br \/>Pot\u00e1p\u011b\u010d za\u017e\u00edval i nad\u00e1le ob\u010dasn\u00e9 brn\u011bn\u00ed v prav\u00e9 noze. Nejpatrn\u011bj\u0161\u00ed bylo b\u011bhem \u0161estihodinov\u00e9 j\u00edzdy autem dva dny po hyperbarick\u00e9 terapii. V\u011bdom si mo\u017en\u00fdch probl\u00e9m\u016f, postupoval p\u0159esn\u011b podle rad l\u00e9ka\u0159e a do letadla nastoupil a\u017e po t\u00fddnu. T\u00fdden po hyperbarick\u00e9 terapii znovu nav\u00e1zal na sv\u00e9 v\u00fdcvikov\u00e9 leteck\u00e9 hodiny. Jeho prvn\u00ed let (v jednomotorov\u00e9m letadle s kabinou s atmosf\u00e9rick\u00fdm tlakem) se uskute\u010dnil do maxim\u00e1ln\u00ed v\u00fd\u0161ky 340 metr\u016f a pot\u00e1p\u011b\u010d neza\u017eil \u017e\u00e1dn\u00e9 op\u011btovn\u00e9 p\u0159\u00edznaky. O dva dny pozd\u011bji let\u011bl p\u0159ibli\u017en\u011b se stejn\u00fdmi parametry a op\u011bt bez p\u0159\u00edznak\u016f. P\u0159i t\u0159et\u00edm letu (do maxim\u00e1ln\u00ed v\u00fd\u0161ky 580 metr\u016f) se mu p\u0159\u00edznaky vr\u00e1tily se stejnou intenzitou jako p\u016fvodn\u011b. N\u00e1sledn\u011b se dostavily i v noci. Vr\u00e1til se do nemocnice, kde jej vy\u0161et\u0159il neurolog a ten rozhodl o vy\u0161et\u0159en\u00ed magnetickou rezonanc\u00ed (MRI).<br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">P\u0159i tomto vy\u0161et\u0159en\u00ed l\u00e9ka\u0159i zjistili vyh\u0159eznutou plot\u00e9nku mezi \u010dtvrt\u00fdm a p\u00e1t\u00fdm bedern\u00edm obratlem. Neurolog a hyperbari\u010dt\u00ed specialist\u00e9 diskutovali o mo\u017enosti, zdali mohlo b\u00fdt vyh\u0159eznut\u00ed plot\u00e9nky v\u00fdsledkem dekompresn\u00ed nemoci a do\u0161li k z\u00e1v\u011bru, \u017ee to je velmi nepravd\u011bpodobn\u00e9. Brn\u011bn\u00ed se dostavovalo je\u0161t\u011b m\u011bs\u00edc po p\u016fvodn\u00edm zran\u011bn\u00ed. Hyperbarick\u00fd person\u00e1l se rozhodl pro dal\u0161\u00ed terapii, tentokr\u00e1t podle tabulky \u010d. 9. Ani tato dodate\u010dn\u00e1 l\u00e9\u010dba v\u0161ak p\u0159etrv\u00e1vaj\u00edc\u00ed p\u0159\u00edznaky nezm\u011bnila. Pot\u00e1p\u011b\u010d zah\u00e1jil program fyzioterapeutick\u00fdch cvi\u010den\u00ed. O t\u0159i m\u011bs\u00edce pozd\u011bji (po ukon\u010den\u00ed zm\u00edn\u011bn\u00e9 fyzioterapie) se vr\u00e1til k pot\u00e1p\u011bn\u00ed. \u017d\u00e1dn\u00e9 dal\u0161\u00ed p\u0159\u00edznaky se ji\u017e neobjevily.<\/p>\n<p style=\"text-align: justify;\"><strong>Shrnut\u00ed<\/strong><br \/>Asi se nikdy pln\u011b nezjist\u00ed, jakou roli sehr\u00e1la jeho z\u00e1da p\u0159i popsan\u00e9m pot\u00e1p\u011b\u010dsk\u00e9m zran\u011bn\u00ed. Dodr\u017eov\u00e1n\u00ed v\u0161eobecn\u00fdch bezpe\u010dnostn\u00edch pravidel v\u0161ak m\u016f\u017ee pomoci sn\u00ed\u017eit riziko DCS a m\u016f\u017ee usnadnit diagnostiku dal\u0161\u00edch mo\u017en\u00fdch p\u0159\u00ed\u010din\/zran\u011bn\u00ed. Tento pot\u00e1p\u011b\u010d ud\u011blal velmi dob\u0159e, \u017ee se choval p\u0159esn\u011b podle v\u0161ech rad a doporu\u010den\u00ed, a proto jsme vybrali tento p\u0159\u00edpad jako vhodn\u00fd pro pou\u010den\u00ed ostatn\u00edch.<\/p>\n<ol>\n<li style=\"text-align: justify;\">Pot\u00e1p\u011b\u010dsk\u00e9 po\u010d\u00edta\u010de jsou naprogamovan\u00e9 tak, aby stanovovaly maxim\u00e1ln\u00ed dobu pobytu pod vodou v z\u00e1vislosti na profilu ponoru. Pro maxim\u00e1ln\u00ed bezpe\u010dnost ponoru se zohled\u0148uje cel\u00e1 \u0159ada podm\u00ednek, nap\u0159. pravd\u011bpodobn\u00e1 n\u00e1maha, teplota atd.<\/li>\n<li style=\"text-align: justify;\">Pot\u00e1p\u011b\u010di by se m\u011bli vyh\u00fdbat \u010dast\u00fdm zm\u011bn\u00e1m hloubky, ale povrch dna nebo \u010dlenitost vraku mohou dodr\u017eov\u00e1n\u00ed tohota pravidla znesnadnit. D\u016fkladn\u00e9 p\u0159edchoz\u00ed prostudov\u00e1n\u00ed oblasti pl\u00e1novan\u00fdch ponor\u016f m\u016f\u017ee v\u00fdznamn\u011b sn\u00ed\u017eit pot\u0159ebu zbyte\u010dn\u00e9ho (\u010dast\u00e9ho) m\u011bn\u011bn\u00ed hloubek. Prosp\u011b\u0161n\u00e9 jsou tak\u00e9 del\u0161\u00ed p\u0159est\u00e1vky mezi ponory na povrchu (ty se vyplat\u00ed prakticky v\u017edy), kter\u00e9 umo\u017e\u0148uj\u00ed intenz\u00edvn\u011bj\u0161\u00ed odplyn\u011bn\u00ed a t\u00edm vy\u0161\u0161\u00ed bezpe\u010dnost.<\/li>\n<li style=\"text-align: justify;\">Rovn\u011b\u017e je pot\u0159eba db\u00e1t na dostate\u010dnou ochranu proti nejr\u016fzn\u011bj\u0161\u00edm vn\u011bj\u0161\u00edm vliv\u016fm. Nap\u0159. dopady rozd\u00edln\u00fdch teplot a v\u0161eho, co s t\u00edm souvis\u00ed, jsou v\u00fdznamn\u00e9. K tomuto t\u00e9matu uv\u00e1d\u00edme podrobn\u011bj\u0161\u00ed informace (viz pozn\u00e1mky dole). Zvl\u00e1\u0161t\u011b to plat\u00ed p\u0159i obt\u00ed\u017en\u00fdch podm\u00ednk\u00e1ch a za okolnost\u00ed, kdy m\u016f\u017ee odply\u0148ov\u00e1n\u00ed \u010dinit probl\u00e9my. Z\u00e1kladn\u00ed pravidlo tedy zn\u00ed: Nejhlub\u0161\u00ed ponor jako prvn\u00ed. Profily ponor\u016f tohoto pot\u00e1p\u011b\u010de nejsou nejlep\u0161\u00edm p\u0159\u00edkladem, proto\u017ee u n\u011bj bylo po\u0159ad\u00ed obr\u00e1cen\u00e9 &ndash; nejd\u0159\u00edve do hloubky 20 metr\u016f, potom do 22.<\/li>\n<li style=\"text-align: justify;\">Procvi\u010dujte a udr\u017eujte si sv\u00e9 dovednosti. Z\u00e1kladn\u00ed dovednosti, kter\u00e9 se u\u010dily v kurzech na otev\u0159en\u00e9 vod\u011b, nikdy nepozbyly sv\u00e9 platnosti. Kdy\u017e m\u00e1te v pot\u00e1p\u011bn\u00ed dlouhou p\u0159est\u00e1vku, m\u011bli byste absolvovat opakovac\u00ed kurz, abyste si sv\u00e9 ztracen\u00e9 schopnosti a dovednosti znovu o\u017eivili. Dovednosti &ndash; hlavn\u011b v situac\u00edch, kdy se pot\u00e1p\u011b\u010d dostane do kritick\u00e9 situace a za\u017e\u00edv\u00e1 stres &ndash; mus\u00ed b\u00fdt druhou p\u0159irozenost\u00ed, mus\u00ed b\u00fdt &ldquo;v krvi&rdquo;.<\/li>\n<\/ol>\n<p style=\"text-align: justify;\">A kone\u010dn\u011b &ndash; v\u0161ichni jsme se u\u010dili: &ldquo;Ponor si napl\u00e1nuj a podle pl\u00e1nu se tak\u00e9 pot\u00e1p\u011bj.&rdquo; N\u011bkolik minut str\u00e1ven\u00fdch pl\u00e1nov\u00e1n\u00edm ponor\u016f m\u016f\u017ee zabr\u00e1nit nejr\u016fzn\u011bj\u0161\u00edm probl\u00e9m\u016fm, a tak n\u00e1s ovlivnit na dlouhou dobu.
