Sotva se tady na severn\u00ed polokouli dostav\u00ed tepl\u00e9 po\u010das\u00ed, za\u010dne mnoho lid\u00ed okam\u017eit\u011b jezdit na kole, chodit po hor\u00e1ch nebo se pot\u00e1p\u011bt. A v\u016fbec si nev\u0161\u00edmaj\u00ed kil nav\u00edc, tepl\u00e9 po\u010das\u00ed je prost\u011b l\u00e1k\u00e1 ven k \u010dinnostem, kter\u00fdch se museli na \u0159adu m\u011bs\u00edc\u016f vzd\u00e1t. Pro mnoho pot\u00e1p\u011b\u010d\u016f znamen\u00e1 zima m\u00e9n\u011b cvi\u010den\u00ed i pot\u00e1p\u011bn\u00ed, vydatn\u011bj\u0161\u00ed j\u00eddla a sklon nabrat dal\u0161\u00ed kila, zpravidla formou ne\u017e\u00e1douc\u00edho tuku. <\/p>\n<p style=\"text-align: justify;\">Jestli\u017ee v\u00e1m tento zimn\u00ed sc\u00e9n\u00e1\u0159 p\u0159ipom\u00edn\u00e1 v\u00e1s osobn\u00ed p\u0159\u00edpad, v\u011bzte, \u017ee nejste sami. T\u00e9m\u011b\u0159 dv\u011b t\u0159etiny dosp\u011bl\u00fdch Ameri\u010dan\u016f se \u0159ad\u00ed do kategorie s nadv\u00e1hou a 30,5 procenta je obezn\u00edch (podle pr\u016fzkumu N\u00e1rodn\u00edho \u00fastavu pro zdrav\u00ed a v\u00fd\u017eivu (americk\u00e1 zkratka: NHANES) z let 1999-2000.<br \/> T\u0159icet p\u011bt procent dosp\u011bl\u00fdch a 14 procent d\u011bt\u00ed v\u00e1\u017e\u00ed podstatn\u011b v\u00edce ne\u017e by m\u011bli. Hlavn\u00edmi &ldquo;vin\u00edky&rdquo; jsou p\u0159ej\u00edd\u00e1n\u00ed a nedostatek pohybu.<\/p>\n<p style=\"text-align: justify;\"><strong>Nadv\u00e1ha a obezita<\/strong><br \/> D\u016fle\u017eit\u00e9 je rozli\u0161ovat mezi nadv\u00e1hou a obezitou. \u010clov\u011bk m\u00e1 nadv\u00e1hu, jestli\u017ee nep\u0159ekra\u010duje o v\u00edce ne\u017e 20 procent svou t\u011blesnou hmotnost podle tabulky pro ide\u00e1ln\u00ed pom\u011br &ldquo;hmotnost-v\u00fd\u0161ka&rdquo; (nap\u0159. podle tabulky vydan\u00e9 americkou Metropolitn\u00ed \u017eivotn\u00ed poji\u0161\u0165ovnou). <br \/> Nej\u010dast\u011bj\u0161\u00ed definice obezity zn\u00ed tak, \u017ee obezn\u00ed \u010dlov\u011bk m\u00e1 procento t\u011blesn\u00e9ho tuku vy\u0161\u0161\u00ed ne\u017e 20 procent sv\u00e9 celkov\u00e9 t\u011blesn\u00e9 hmotnosti. Toto procento t\u011blesn\u00e9ho tuku se zpravidla m\u011b\u0159\u00ed jako tlou\u0161\u0165ka tuku v ko\u017en\u00ed \u0159ase (i kdy\u017e ne cel\u00e1 nam\u011b\u0159en\u00e1 tlou\u0161\u0165ka je pouze tuk), co\u017e se zji\u0161\u0165uje kaliperem na p\u0159esn\u011b ur\u010den\u00fdch m\u00edstech, nap\u0159. t\u011bsn\u011b pod lopatkou, na stehn\u011b, na tricepsu a na b\u0159ichu. <\/p>\n<p style=\"text-align: justify;\">Dal\u0161\u00ed zp\u016fsob m\u011b\u0159en\u00ed obezity je pomoci tzv. indexu t\u011blesn\u00e9 hmotnosti (BMI). Index t\u011blesn\u00e9 hmotnosti byl vyvinut N\u00e1rodn\u00edm \u00fastavem pro zdrav\u00ed a vypo\u010d\u00edt\u00e1v\u00e1 se pomoci matematick\u00e9ho vzorce, kter\u00fd je rovn\u011b\u017e zalo\u017een na principu porovn\u00e1v\u00e1n\u00ed v\u00fd\u0161ky a v\u00e1hy hodnocen\u00e9 osoby. V\u00fdpo\u010det BMI se provede n\u00e1sledovn\u011b: V\u00e1ha \u010dlov\u011bka v kilogramech se d\u011bl\u00ed druhou mocninou jeho v\u00fd\u0161ky v metrech. (BMI=kg\/m2).<br \/> Nap\u0159. BMI od 18,5 do 24,9 se pova\u017euje za norm\u00e1ln\u00ed, ale BMI od 30,0 do 39 znamen\u00e1 obezitu. Zat\u00edmco nadv\u00e1ha je probl\u00e9m, obezita m\u016f\u017ee zna\u010dn\u011b prohloubit zdravotn\u00ed pot\u00ed\u017ee, jak\u00fdmi jsou vysok\u00fd krevn\u00ed tlak (hypertenze), diabetes a zrychlen\u00e1 ateroskleroza (t.j. tvorba tukov\u00fdch, vazivov\u00fdch a ateromov\u00fdch pl\u00e1t\u016f ve vnit\u0159n\u00ed vrstv\u011b tepenn\u00e9 st\u011bny, co\u017e sni\u017euje pru\u017enost tepen). Proto se u ob\u00e9zn\u00edch lid\u00ed zvy\u0161uje riziko srde\u010dn\u00edho infarktu a mozkov\u00e9 mrtvice a n\u00e1sledkem toho se u nich sni\u017euje p\u0159edpokl\u00e1dan\u00e1 d\u00e9lka \u017eivota.<\/p>\n<p style=\"text-align: justify;\">Nadv\u00e1ha a obezita jsou zn\u00e1m\u00e9 rizikov\u00e9 faktory pro:<\/p>\n<ul>\n<li style=\"text-align: justify;\">diabetes;<\/li>\n<li style=\"text-align: justify;\">srde\u010dn\u00ed onemocn\u011bn\u00ed;<\/li>\n<li style=\"text-align: justify;\">mozkovou mrtvici;<\/li>\n<li style=\"text-align: justify;\">hypertenzi;<\/li>\n<li style=\"text-align: justify;\">onemocn\u011bn\u00ed \u017elu\u010dn\u00edku;<\/li>\n<li style=\"text-align: justify;\">osteoartritidu (t.j. degeneraci chrupavek a kosti v oblasti kloub\u016f);<\/li>\n<li style=\"text-align: justify;\">sp\u00e1nkovou apnoe (t.j. kr\u00e1tkodobou do\u010dasnou z\u00e1stavu d\u00fdch\u00e1n\u00ed p\u0159i sp\u00e1nku) a jin\u00e9 d\u00fdchac\u00ed pot\u00ed\u017ee;<\/li>\n<li style=\"text-align: justify;\">n\u011bkter\u00e9 druhy rakoviny (d\u011blohy, prsu, tlust\u00e9ho st\u0159eva, ledvin a \u017elu\u010dn\u00edku).<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Obezita se rovn\u011b\u017e spojuje s:<\/p>\n<ul>\n<li style=\"text-align: justify;\">vysokou hladinou krevn\u00edho cholesterolu;<\/li>\n<li style=\"text-align: justify;\">pot\u00ed\u017eemi b\u011bhem t\u011bhotenstv\u00ed;<\/li>\n<li style=\"text-align: justify;\">menstrua\u010dn\u00edmi poruchami;<\/li>\n<li style=\"text-align: justify;\">hirsutizmem (t.j. nadm\u011brn\u00fdm ochlupen\u00edm na t\u011ble a v obli\u010deji);<\/li>\n<li style=\"text-align: justify;\">stresovou inkontinenc\u00ed (t.j. \u00fanikem mo\u010di zp\u016fsoben\u00fdm slab\u00fdm svalstvem p\u00e1nevn\u00edho dna);<\/li>\n<li style=\"text-align: justify;\">psychologick\u00fdmi poruchami, nap\u0159. depresemi;<\/li>\n<li style=\"text-align: justify;\">zv\u00fd\u0161en\u00fdm rizikem p\u0159i jak\u00e9mkoliv chirurgick\u00e9m z\u00e1kroku.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Obezita a s n\u00ed spojen\u00e1 zdravotn\u00ed rizika mohou rovn\u011b\u017e u\u010dinit pot\u00e1p\u011bn\u00ed m\u00e9n\u011b bezpe\u010dn\u00fdm jak pro samotn\u00e9ho ob\u00e9zn\u00edho pot\u00e1p\u011b\u010de, tak i pro jeho partnera.<br \/> Chirurgick\u00e9 zp\u016fsoby sni\u017eov\u00e1n\u00ed t\u011blesn\u00e9 hmotnosti I kdy\u017e l\u00e9ka\u0159i i cel\u00e9 organizace, nap\u0159. Americk\u00e1 asociace pro srde\u010dn\u00ed onemocn\u011bn\u00ed nebo CDC (Centers for Decease Control and Prevention), doporu\u010duj\u00ed celou \u0159adu zdrav\u00fdch diet a pohybov\u00fdch cvi\u010den\u00ed, je smutnou skute\u010dnost\u00ed, \u017ee obezita z\u016fst\u00e1v\u00e1 st\u00e1le velk\u00fdm probl\u00e9mem. A i kdy\u017e je mnohem lep\u0161\u00ed hl\u00eddat si svou hmotnost a neztloustnout, n\u011bkte\u0159\u00ed jedinci se rozhodli, \u017ee kdy\u017e u\u017e se do stavu obezity dostali, nebudou se zdr\u017eovat dietami a rad\u011bji nasad\u00ed rychlej\u0161\u00ed \u0159e\u0161en\u00ed. Lid\u00e9 maj\u00edc\u00ed v\u00e1\u017en\u00e9 probl\u00e9my se svou nadm\u011brnou t\u011blesnou hmotnost\u00ed, kterou nedok\u00e1\u017e\u00ed zvl\u00e1dnout reduk\u010dn\u00ed dietou a programem t\u011blesn\u00fdch cvi\u010den\u00ed, se \u010dasto st\u00e1vaj\u00ed z\u00e1jemci a uchaze\u010di o extr\u00e9mn\u011bj\u0161\u00ed z\u00e1sahy. <\/p>\n<p style=\"text-align: justify;\">Posledn\u00ed dobou se st\u00e1le v\u00edce lid\u00ed rozhoduje pro r\u016fzn\u00e9 chirurgick\u00e9 z\u00e1kroky, jak\u00fdmi jsou nap\u0159. gastrick\u00fd bypas nebo bariatrick\u00fd chirurgick\u00fd z\u00e1krok (angl. bariatrics = l\u00e9\u010dba obezity) kv\u016fli sn\u00ed\u017een\u00ed t\u011blesn\u00e9 hmotnosti a pro jej\u00ed n\u00e1slednou kontrolu. Podle Americk\u00e9 spole\u010dnosti pro bariatrickou chirurgii podstoupilo koncem 90-t\u00fdch let minul\u00e9ho stolet\u00ed v USA takov\u00e9 z\u00e1kroky 16 000 jedinc\u016f. V roce 2003 absolvovalo n\u011bjakou formu bariatrick\u00e9ho chirurgick\u00e9ho z\u00e1kroku ji\u017e v\u00edce ne\u017e 103 000 pacient\u016f. Chirurgick\u00e9 z\u00e1kroky pro sni\u017eov\u00e1n\u00ed t\u011blesn\u00e9 hmotnosti se pova\u017euj\u00ed za velk\u00e9 operace a nesou sebou zna\u010dn\u00e9 riziko i mo\u017enost dlouhodob\u00fdch n\u00e1sledk\u016f. Proto by m\u011bli pot\u00e1p\u011b\u010di v\u011bd\u011bt, jak se tyto postupy slu\u010duj\u00ed nebo neslu\u010duj\u00ed s mo\u017enost\u00ed v\u011bnovat se pot\u00e1p\u011bn\u00ed.<br \/> &nbsp;<\/p>\n<p style=\"text-align: justify;\">V\u0161eobecn\u011b se doporu\u010duje n\u011bkolik metod. Podrobn\u00e9 informace o ka\u017ed\u00e9 z nich jsou dostupn\u00e9 z r\u016fzn\u00fdch zdroj\u016f, hlavn\u011b by se v\u0161ak m\u011bly z\u00edsk\u00e1vat od profesion\u00e1ln\u00edch zdravotn\u00edk\u016f nebo p\u0159\u00edmo od p\u0159\u00edslu\u0161n\u00e9ho chirurga. Stru\u010dn\u011b \u0159e\u010deno, existuj\u00ed dva hlavn\u00ed postupy, p\u0159i\u010dem\u017e u ka\u017ed\u00e9ho z nich se jedn\u00e1 o zmen\u0161en\u00ed velikosti \u017ealudku.<br \/> Gastrick\u00fd bypas Roux en Y Prvn\u00ed postup spo\u010d\u00edv\u00e1 na tzv. &quot;malabsorpci&rdquo; (t.j poru\u0161e tr\u00e1ven\u00ed). V podstat\u011b jde o to, \u017ee se p\u0159eru\u0161\u00ed a v\u00fdznamn\u011b zkr\u00e1t\u00ed tenk\u00e9 st\u0159evo, pot\u00e9 se se\u0161ije do tvaru p\u00edsmene &ldquo;Y&rdquo; a t\u00edm se zkr\u00e1t\u00ed cel\u00fd tr\u00e1vic\u00ed trakt.<br \/> (Pozn\u00e1mka: V\u011bt\u0161ina tr\u00e1ven\u00ed se odehr\u00e1v\u00e1 ve st\u0159evech, ne v \u017ealudku.) Zkr\u00e1cen\u00ed st\u0159ev sni\u017euje mno\u017estv\u00ed kalori\u00ed, cukr\u016f i tuk\u016f, kter\u00e9 by absorboval norm\u00e1ln\u00ed tr\u00e1vic\u00ed trakt. To pacient\u016fm umo\u017e\u0148uje j\u00edst norm\u00e1ln\u00ed j\u00eddla&nbsp; &ndash; jak co do mno\u017estv\u00ed, tak i rozmanitosti. Absorbuje se v\u0161ak m\u00e9n\u011b potravy, proto pacient ztr\u00e1c\u00ed v\u00e1hu. <\/p>\n<p style=\"text-align: justify;\">V takov\u00e9m p\u0159\u00edpad\u011b v\u0161ak nedoch\u00e1z\u00ed k dostate\u010dn\u00e9 absorpci ani n\u011bkter\u00fdch d\u016fle\u017eit\u00fdch \u017eivin, vitamin\u016f a dal\u0161\u00edch nezbytn\u00fdch slo\u017eek potravy (hlavn\u011b b\u00edlkovin, n\u011bkter\u00fdch tuk\u016f, tukem rozpustn\u00fdch vitamin\u016f, vitamin\u016f A, D, E a K, \u017eeleza, v\u00e1pn\u00edku a vitaminu B12).<br \/> Pacienti mohou p\u0159edej\u00edt nedostate\u010dnosti zm\u00edn\u011bn\u00fdch d\u016fle\u017eit\u00fdch slo\u017eek potravy p\u0159\u00edjmem p\u0159\u00edslu\u0161n\u00fdch potravinov\u00fdch dopl\u0148k\u016f. Tento chirurgick\u00fd z\u00e1krok m\u00e1 za n\u00e1sledek zdravotn\u00ed\/t\u011blesn\u00fd stav zn\u00e1m\u00fd jako &quot;dumpingov\u00fd syndrom.&quot; \u017daludek i st\u0159eva posunuj\u00ed stravu s\u00e9ri\u00ed vlnovit\u00fdch kontrakc\u00ed, \u010demu\u017e se \u0159\u00edk\u00e1 peristaltika. Dumpingov\u00fd syndrom nast\u00e1v\u00e1 tehdy, kdy\u017e \u017ealudek rychle vypr\u00e1zdn\u00ed sv\u016fj obsah do jejuna (la\u010dn\u00edku), co\u017e je \u010d\u00e1st tenk\u00e9ho st\u0159eva mezi duodenem (dvan\u00e1ctn\u00edkem) a ileem (kone\u010dn\u00fdm \u00fasekem tenk\u00e9ho st\u0159eva, ky\u010deln\u00edkem). N\u00e1sledkem toho se dostavuj\u00ed p\u0159\u00edznaky jako zrychlen\u00fd srde\u010dn\u00ed tep, siln\u00e9 pocen\u00ed, zkr\u00e1cen\u00e1 doba\/schopnost soust\u0159ed\u011bn\u00ed se, bolesti\/k\u0159e\u010de v b\u0159i\u0161e, z\u00e1vrat\u011b, pr\u016fjem a n\u00e1valy. <\/p>\n<p style=\"text-align: justify;\">Tyto p\u0159\u00edznaky se mohou dostavovat od 30 minut a\u017e do 3 hodin po j\u00eddle. Kdyby se dostavily u pot\u00e1p\u011b\u010de pod vodou, mohlo by to m\u00edt nebezpe\u010dn\u00e9 n\u00e1sledky. Proto se v\u011bt\u0161ina l\u00e9ka\u0159\u016f shoduje na z\u00e1v\u011bru, \u017ee pacient, kter\u00fdm m\u00e1 sklon k &ldquo;dumpingov\u00e9mu syndromu&rdquo;, by se nem\u011bl pot\u00e1p\u011bt d\u0159\u00edve ne\u017e 3 hodiny po j\u00eddle.<br \/> Jednou z variant malabsorp\u010dn\u00ed metody je tzv. &quot;duoden\u00e1ln\u00ed switch,&quot; p\u0159i kter\u00e9 se \u010d\u00e1st bypasovan\u00e9ho tenk\u00e9ho st\u0159eva znovu p\u0159ipoj\u00ed k tr\u00e1vic\u00edmu traktu, ov\u0161em na jin\u00e9m m\u00edst\u011b. T\u00edm se dosahuje stejn\u00fdch malabsorp\u010dn\u00edch v\u00fdsledk\u016f. Pou\u017eit\u00edm t\u00e9to metody se eliminuje dumpingov\u00fd syndrom, ale z\u016fst\u00e1v\u00e1 mopot\u0159eba dod\u00e1vat potravinov\u00e9 dopl\u0148ky a sledovat zdravotn\u00ed stav cel\u00e9ho organizmu.<\/p>\n<p style=\"text-align: justify;\"><strong>Redukce \u017ealudku<\/strong><br \/> Vertik\u00e1ln\u00ed gastroplastika (band\u00e1\u017e \u017ealudku) Existuje i jin\u00e1 a dokonce je\u0161t\u011b restriktivn\u011bj\u0161\u00ed metoda pro sn\u00ed\u017een\u00ed velikosti \u017ealudku. Po tomto z\u00e1kroku m\u016f\u017ee z\u016fstat pacientovi objem \u017ealudku o velikosti 1 a\u017e dvou pol\u00e9vkov\u00fdch l\u017eic. Tato metoda drasticky sni\u017euje mo\u017estv\u00ed j\u00eddla, kter\u00e9 m\u016f\u017ee pacient p\u0159ijmout najednou, av\u0161ak tr\u00e1ven\u00ed prob\u00edh\u00e1 norm\u00e1ln\u011b a probl\u00e9my s nedostate\u010dnou v\u00fd\u017eivou (nap\u0159. an\u00e9mie nebo osteopor\u00e9za) prakticky neexistuj\u00ed. Pacient ani nemus\u00ed b\u00fdt pod n\u011bjak\u00fdm pr\u016fb\u011b\u017en\u00fdm zdravotn\u00edm dozorem a nen\u00ed nucen p\u0159ij\u00edmat potravinov\u00e9 dopl\u0148ky.<br \/> Kdo se v\u0161ak rozhodne pro tuto metodu, mus\u00ed pe\u010dliv\u011b volit j\u00eddlo, kter\u00e9 bude j\u00edst: Proto\u017ee absorpce nen\u00ed nijak ovlivn\u011bna \u010di zm\u011bn\u011bna, ve\u0161ker\u00e1 potrava obsahuj\u00edc\u00ed vysok\u00e9 mno\u017estv\u00ed kalori\u00ed, cukr\u016f a tuk\u016f bude m\u00edt na t\u011blo stejn\u00fd \u00fa\u010dinek, jak tomu bylo p\u0159ed operac\u00ed. A jestli\u017ee pacient p\u0159ijme p\u0159\u00edli\u0161 mnoho j\u00eddla nebo velk\u00e9 kusy, m\u016f\u017ee u n\u011bj doj\u00edt ke zvracen\u00ed.  <\/p>\n<p style=\"text-align: justify;\">Zdravotn\u00ed stavy po proveden\u00fdch operac\u00edch U n\u011bkter\u00fdch pacient\u016f doch\u00e1z\u00ed po operaci k chronick\u00e9 regurgitaci (zp\u011btn\u00e9mu toku) kyselin. Zp\u011btn\u00fd tok tekutin a \u017ealude\u010dn\u00edch \u0161\u0165\u00e1v m\u016f\u017ee p\u016fsobit pot\u00e1p\u011b\u010d\u016fm zna\u010dn\u00e9 probl\u00e9my, hlavn\u011b v poloze &ldquo;hlavou dol\u016f&rdquo;, kter\u00e1 je \u010dast\u00e1 p\u0159i sestupu. Probl\u00e9my regurgitace a zvracen\u00ed v\u0161ak lze zvl\u00e1dnout spr\u00e1vn\u00fdmi stravovac\u00edmi a dietn\u00edmi n\u00e1vyky a u\u017e\u00edv\u00e1n\u00edm vhodn\u00fdch l\u00e9k\u016f.<br \/> Pro pot\u00e1p\u011b\u010de p\u0159edstavuje velmi specifick\u00fd probl\u00e9m tzv. gastrointestin\u00e1ln\u00ed barotrauma (barotrauma tr\u00e1vic\u00edho traktu). B\u011bhem v\u00fdstupu toti\u017e expanduj\u00ed plynov\u00e9 kapsy obsa\u017een\u00e9 v gastrointestin\u00e1ln\u00edm traktu.<br \/> A jestli\u017ee je plyn zachycen v n\u011bjak\u00e9m neprostupn\u011b ucpan\u00e9m st\u0159evu, mohlo by doj\u00edt k jeho protr\u017een\u00ed. L\u00e9ka\u0159\u0161t\u00ed specialist\u00e9 na pot\u00e1p\u011bn\u00ed v\u0161ak tvrd\u00ed, \u017ee u pot\u00e1p\u011b\u010d\u016f, kte\u0159\u00ed absolvovali bariatrick\u00fd chirurgick\u00fd z\u00e1krok bez n\u00e1sledn\u00fdch komplikac\u00ed, neexistuje ani zv\u00fd\u0161en\u00e9 riziko gastrointestin\u00e1ln\u00edho barotraumatu.<\/p>\n<p style=\"text-align: justify;\"><strong>Zp\u011bt k pot\u00e1p\u011bn\u00ed<\/strong><br \/> Za jak dlouho po bariatrick\u00e9 operaci je tedy mo\u017en\u00e9 se znovu pot\u00e1p\u011bt?
