Zpravidla jde o bublinky inertn&iacute;ho plynu dus&iacute;ku. DCS p\u0159edstavuje zdravotn&iacute; riziko souvisej&iacute;c&iacute; s pot&aacute;p\u011bn&iacute;m, p\u0159i\u010dem\u017e projev\u016f t&eacute;to nemoci m\u016f\u017ee b&yacute;t &scaron;irok&aacute; &scaron;k&aacute;la &ndash; od vyr&aacute;\u017eky, p\u0159es bolesti kloub\u016f r\u016fzn&eacute; intenzity, a\u017e po celo\u017eivotn&iacute; trval&eacute; ochrnut&iacute; a v nejhor&scaron;&iacute;m p\u0159&iacute;pad\u011b i &uacute;mrt&iacute;. Jestli\u017ee se inertn&iacute; plyn, kter&yacute; nasytil t\u011blesn&eacute; tk&aacute;n\u011b b\u011bhem zv&yacute;&scaron;en&eacute;ho okoln&iacute;ho tlaku, uvol\u0148uje p\u0159i sni\u017eov&aacute;n&iacute; tlaku p\u0159&iacute;li&scaron; rychle, mohou se z n\u011bj v t\u011ble vytvo\u0159it cirkuluj&iacute;c&iacute; nebo tk&aacute;\u0148ov&eacute; bublinky, kter&eacute; hroz&iacute; zablokov&aacute;n&iacute;m c&eacute;v, zvl&aacute;&scaron;t\u011b pak mal&yacute;ch kapil&aacute;r. Nastane-li tato situace v mozku, m&iacute;&scaron;e nebo srdci, mohou b&yacute;t n&aacute;sledky velmi v&aacute;\u017en&eacute; a\u017e katastrof&aacute;ln&iacute;. Lep&scaron;&iacute; pochopen&iacute; a prevence tohoto jevu hraj&iacute; p\u0159i vyvarov&aacute;n&iacute; se tomuto nebezpe\u010d&iacute; a zm&iacute;rn\u011bn&iacute; mo\u017en&yacute;ch n&aacute;sledk\u016f kl&iacute;\u010dovou roli.<\/p>\n<p style=\"text-align: justify;\">V roce 2009 zah&aacute;jila skupina 14 nad&scaron;en&yacute;ch v&yacute;zkumn&yacute;ch pracovn&iacute;k\u016f pr&aacute;ci na projektu nazvan&eacute;m PHYPODE (PHYsioPathology Of Decompression), kter&yacute; byl sponzorov&aacute;n Evropskou uni&iacute; v r&aacute;mci inciativy Marie Curie Initial Training Networks, s c&iacute;lem prozkoumat DCS hloub\u011bji ne\u017e jen z hlediska pouh&yacute;ch fyziologick&yacute;ch proces\u016f.<\/p>\n<p style=\"text-align: justify;\">Nyn&iacute; byl po 4 letech zm&iacute;n\u011bn&yacute; projekt ukon\u010den. S c&iacute;lem zve\u0159ejnit z&iacute;skan&eacute; poznatky zorganizovala DAN (Divers Alert Network Europe) v bruselsk&eacute;m ISEK 20. prosince 2014 jednodenn&iacute; konferenci pod n&aacute;zvem &ldquo;The Science of Diving&rdquo;.&nbsp;<\/p>\n<p style=\"text-align: justify;\"><strong><span style=\"font-size: 18px;\">V\u011bdeck&eacute; p\u0159edpoklady &nbsp;<\/span><\/strong><\/p>\n<p style=\"text-align: justify;\">Jeliko\u017e se n\u011bkdy vyskytuj&iacute; p\u0159&iacute;pady DCS i bez zjevn&eacute;ho pochyben&iacute; v technice pot&aacute;p\u011bn&iacute; a nelze je vysv\u011btlit pouh&yacute;m p\u0159esycen&iacute;m tk&aacute;n&iacute;, panuje v&scaron;eobecn&eacute; p\u0159esv\u011bd\u010den&iacute;, \u017ee dekompresn&iacute; modely, kter&eacute; se pou\u017e&iacute;vaj&iacute; v sou\u010dasn&yacute;ch pot&aacute;p\u011b\u010dsk&yacute;ch po\u010d&iacute;ta\u010d&iacute;ch, nejsou dostate\u010dn\u011b bezpe\u010dn&eacute;, m\u011bly by se znovu p\u0159ezkoumat a p\u0159&iacute;padn\u011b poopravit. D&aacute;le existuje p\u0159edpoklad, \u017ee v p\u0159&iacute;pad\u011b DCS se nejedn&aacute; o \u010dist\u011b fyzik&aacute;ln&iacute; jev, n&yacute;br\u017e tak&eacute; o komplexn&iacute; patofyziologick&yacute; proces ovlivn\u011bn&yacute; a\/nebo vyvolan&yacute; r\u016fzn&yacute;mi faktory, kter&eacute; se mohou u r\u016fzn&yacute;ch lid&iacute; li&scaron;it. To vede k z&aacute;v\u011bru, \u017ee riziko rozvoje DCS je u ka\u017ed&eacute;ho pot&aacute;p\u011b\u010de individu&aacute;ln&iacute;. Proto se budouc&iacute; v&yacute;zkumy v t&eacute;to oblasti zam\u011b\u0159&iacute; na v&yacute;voj dokonalej&scaron;&iacute; technologie pou\u017eiteln&eacute; p\u0159i pot&aacute;p\u011bn&iacute; v re&aacute;ln&eacute;m \u010dase. Ide&aacute;ln&iacute;m v&yacute;sledkem by bylo vytvo\u0159en&iacute; jak&eacute;hosi &bdquo;bionick&eacute;ho&ldquo; pot&aacute;p\u011b\u010de vybaven&eacute;ho &bdquo;pot&aacute;p\u011b\u010dsk&yacute;m superpo\u010d&iacute;ta\u010dem&rdquo; s nov&yacute;m algoritmem, kter&yacute; by dok&aacute;zal v re&aacute;ln&eacute;m \u010dase (tj. p\u0159ed ponorem, b\u011bhem ponoru a po ponoru) sledovat zdravotn&iacute; &uacute;daje p\u0159&iacute;slu&scaron;n&eacute;ho jedince.<\/p>\n<p style=\"text-align: justify;\"><strong><span style=\"font-size: 18px;\">V\u011bdeck&eacute; v&yacute;sledky &nbsp;<\/span><\/strong><\/p>\n<p style=\"text-align: justify;\">Pro lep&scaron;&iacute; pozn&aacute;n&iacute; mnoha fyziologick&yacute;ch faktor\u016f a mechanism\u016f bylo zapot\u0159eb&iacute; pou\u017e&iacute;t multifaktorov&yacute; p\u0159&iacute;stup. Ka\u017ed&yacute; v&yacute;zkum v t&eacute;to oblasti, kter&yacute; pom\u016f\u017ee prohloubit pochopen&iacute; mechanism\u016f a proces\u016f vedouc&iacute;ch k DCS, je d\u016fle\u017eit&yacute;m krokem k v\u011bt&scaron;&iacute; bezpe\u010dnosti pot&aacute;p\u011bn&iacute;, co\u017e bylo v\u017edy sou\u010d&aacute;st&iacute; posl&aacute;n&iacute; DAN. Proto tak&eacute; nadace DAN vytvo\u0159ila v roce 1994 laborato\u0159 DSL (Diving Safety Laboratory) s c&iacute;lem z&iacute;skat a shrom&aacute;\u017edit co nejv&iacute;ce &uacute;daj\u016f o uskute\u010dn\u011bn&yacute;ch ponorech, v\u010detn\u011b &uacute;daj\u016f o pot&aacute;p\u011b\u010dsk&yacute;ch nehod&aacute;ch. Pod veden&iacute;m profesora A. Marroniho, prezidenta DAN Europe a jednoho z p\u0159edn&iacute;ch v\u011bdc\u016f z&uacute;\u010dastn\u011bn&yacute;ch na projektu PHYPODE, se tyto &uacute;daje analyzovaly pomoc&iacute; epidemiologick&eacute; anal&yacute;zy, aby se zjistily rizikov&eacute; faktory a jejich znaky.<\/p>\n<p style=\"text-align: justify;\">V roce 2014 byly v datab&aacute;zi DSL sesb&iacute;r&aacute;ny &uacute;daje o 39 944 ponorech uskute\u010dn\u011bn&yacute;ch 2 615 pot&aacute;p\u011b\u010di (2176 mu\u017e\u016f a 439 \u017een v pr\u016fm\u011brn&eacute; v\u011bkov&eacute; skupn\u011b 33-51 let).<\/p>\n<p style=\"text-align: justify;\">Ke zji&scaron;\u0165ov&aacute;n&iacute; produkce cirkuluj&iacute;c&iacute;ch bublinek byla vyu\u017eita technika Dopplerovy sonografie. V\u011bdci prok&aacute;zali, \u017ee bublinky lze zjistit 30 a\u017e 75 minut po v&yacute;stupu na hladinu a vymiz&iacute; asi po 1,5 hodin\u011b. Tak&eacute; se uk&aacute;zalo, \u017ee produkce bublinek se zvy&scaron;ovala s v\u011bkem, ale nez&aacute;le\u017eelo na tom, zda &scaron;lo o mu\u017ee \u010di \u017eeny.<\/p>\n<p style=\"text-align: justify;\">Na konferenci byly prezentov&aacute;ny rovn\u011b\u017e nov&eacute; poznatky t&yacute;kaj&iacute;c&iacute; se <strong>cestov&aacute;n&iacute; letadlem po pot&aacute;p\u011bn&iacute;<\/strong>. Echokardiografick&eacute; vy&scaron;et\u0159en&iacute; pot&aacute;p\u011b\u010d\u016f, vracej&iacute;c&iacute;ch se po t&yacute;denn&iacute; pot&aacute;p\u011b\u010dsk&eacute; dovolen&eacute;, prov&aacute;d\u011bn&eacute; b\u011bhem letu uk&aacute;zalo, \u017ee pot&aacute;p\u011b\u010di, u kter&yacute;ch se zpravidla vyskytuje po ponoru hodn\u011b bublinek, by m\u011bli nastoupit cestu letadlem s v\u011bt&scaron;&iacute;m \u010dasov&yacute;m odstupem (36 &#8211; 48 hodin nam&iacute;sto b\u011b\u017en&yacute;ch 24 hodin po posledn&iacute;m ponoru). &nbsp;<\/p>\n<p style=\"text-align: justify;\">Co se t&yacute;\u010de dal&scaron;&iacute;ch zdravotn&iacute;ch rizik souvisej&iacute;c&iacute;ch s pot&aacute;p\u011bn&iacute;m, dobr&eacute; zpr&aacute;vy se zde dozv\u011bd\u011bli <strong>pot&aacute;p\u011b\u010di diabetici<\/strong>. Nejnov\u011bj&scaron;&iacute; technologie toti\u017e umo\u017e\u0148uj&iacute; pr\u016fb\u011b\u017enou kontrolu hladiny cukru v krvi (BG) i pod vodou. Glykemii je mo\u017eno m\u011b\u0159it za pomoc&iacute; vodot\u011bsn&yacute;ch monitorovac&iacute;ch p\u0159&iacute;stroj\u016f, jejich\u017e \u010didla jsou um&iacute;st\u011bn&aacute; pod mokr&yacute;m oblekem.<\/p>\n<p style=\"text-align: justify;\">V&yacute;zkum <strong>plicn&iacute;ho ed&eacute;mu vyvolan&eacute;ho zadr\u017eov&aacute;n&iacute;m dechu&nbsp;<\/strong>p\u0159inesl nov&eacute; poznatky o genetick&yacute;ch dispozic&iacute;ch, kter&eacute; by mohly \u010dinit n\u011bkter&eacute; pot&aacute;p\u011b\u010de n&aacute;chyln\u011bj&scaron;&iacute; k rozvoji DCS. N\u011bkolik ned&aacute;vno proveden&yacute;ch studi&iacute; nazna\u010dilo, \u017ee ur\u010dit&eacute; genetick&eacute; varianty enzymu e-NOS, kter&eacute; obsahuj&iacute; kyselinu glutamovou nam&iacute;sto kyseliny asparagov&eacute;, vedou k vy&scaron;&scaron;&iacute; produkci oxidu dusnat&eacute;ho (NO), co\u017e m\u016f\u017ee m&iacute;t ochrann&eacute; &uacute;\u010dinky proti DCS.<\/p>\n<p style=\"text-align: justify;\">O <strong>bublink&aacute;ch<\/strong> toho v&iacute;me hodn\u011b, ov&scaron;em p\u0159esn&yacute; mechanismus jejich vzniku z\u016fst&aacute;val a\u017e do ned&aacute;vna dosti nejasn&yacute;. C. Balestra, profesor na bruselsk&eacute; vysok&eacute; &scaron;kole Haute-Ecole Paul Henri Spaak a vedouc&iacute; tamn&iacute; laborato\u0159e integrovan&eacute; fyziologie (Integrative Physiology Lab), \u0159&iacute;dil v&yacute;zkum zam\u011b\u0159en&yacute; na tvorbu bublinek v hyperbarick&eacute; dekompresi, tj. na tvorbu bublinek plynn&eacute;ho dus&iacute;ku v c&eacute;vn&iacute; soustav\u011b pot&aacute;p\u011b\u010de po p\u0159&iacute;strojov&eacute;m pot&aacute;p\u011bn&iacute; a s t&iacute;m souvisej&iacute;c&iacute; fyzik&aacute;ln&iacute; a fyziologick&eacute; procesy. Aby byl jasn\u011b viditeln&yacute; r\u016fst bublinek i jejich hustota, co\u017e by umo\u017enilo p\u0159esn&eacute; sledov&aacute;n&iacute; mechanismu jejich vzniku, pou\u017eil profesor Balestra experiment&aacute;ln&iacute; soupravu ur\u010denou k optick&eacute;mu zaznamen&aacute;v&aacute;n&iacute; sledovan&yacute;ch jev\u016f. Pot&eacute; zkoumal dva odli&scaron;n&eacute; druhy tk&aacute;\u0148ov&yacute;ch povrch\u016f, tj. hydrofiln&iacute; a hydrofobn&iacute;, jin&yacute;mi slovy &#8211; tk&aacute;\u0148 svalovou a tk&aacute;\u0148 tukovou. P\u0159itom zjistil v&yacute;znamn\u011b v&iacute;ce bublinek na tukov&eacute; tk&aacute;ni (hydrofobn&iacute;) ne\u017e na tk&aacute;ni svalov&eacute; (hydrofiln&iacute;). Z\u0159ejm\u011b to souvis&iacute; s v&yacute;skytem tzv. hydrofobn&iacute;ch nuklea\u010dn&iacute;ch bod\u016f, na kter&yacute;ch se mohou bublinky tvo\u0159it. V&scaron;e nasv\u011bd\u010duje tomu, \u017ee s p\u0159ib&yacute;vaj&iacute;c&iacute;m v\u011bkem roste i po\u010det t\u011bchto hydrofobn&iacute;ch bod\u016f v c&eacute;vn&iacute; soustav\u011b \u010dlov\u011bka, co\u017e by vysv\u011btlovalo zvy&scaron;uj&iacute;c&iacute; se riziko mo\u017en&eacute;ho DCS u star&scaron;&iacute;ch jedinc\u016f, ale tak&eacute; vy&scaron;&scaron;&iacute; nebezpe\u010d&iacute; v&yacute;skytu Alzheimerovy choroby, celiakie (nesn&aacute;&scaron;enlivosti lepku), r\u016fzn&yacute;ch alergi&iacute;, diabetu a rakoviny. Hydrofobn&iacute; body existuj&iacute;c&iacute; na vnit\u0159n&iacute; vrstv\u011b c&eacute;v, kde se mohou tvo\u0159it bublinky, maj&iacute; tud&iacute;\u017e dalekos&aacute;hl&yacute; interdisciplin&aacute;rn&iacute; v&yacute;znam a tyto nov\u011b zji&scaron;t\u011bn&eacute; poznatky mohou b&yacute;t p\u0159&iacute;nosem nejen pro pot&aacute;p\u011b\u010de.<\/p>\n<p style=\"text-align: justify;\">Peter Germonpr&eacute; (\u0159editel St\u0159ediska pro hyberbarickou kysl&iacute;kovou terapii ve Vojensk&eacute; nemocnici v Bruselu) zd\u016fraznil d\u016fle\u017eitost tzv. <strong>p\u0159edb\u011b\u017en&eacute; p\u0159&iacute;pravy<\/strong> sest&aacute;vaj&iacute;c&iacute; z \u0159ady krok\u016f, kter&eacute; m\u016f\u017ee (a m&aacute;) ka\u017ed&yacute; pot&aacute;p\u011b\u010d u\u010dinit p\u0159ed ponorem, aby sn&iacute;\u017eil nebezpe\u010d&iacute; DCS. V&scaron;eobecn\u011b se m&aacute; za to, \u017ee existuj&iacute; dva mo\u017en&eacute; mechanismy, jak sn&iacute;\u017eit &nbsp;mno\u017estv&iacute; bublinek: biochemick&yacute;, tj. ovliv\u0148ov&aacute;n&iacute; oxida\u010dn&iacute; z&aacute;n\u011btliv&eacute; reakce vyvolan&eacute; p\u0159&iacute;tomnost&iacute; dekompresn&iacute;ch bublinek v t\u011ble a mechanick&yacute;, tj. sni\u017eov&aacute;n&iacute; mno\u017estv&iacute; bublinkov&yacute;ch &bdquo;jader&ldquo; v c&eacute;vn&iacute;m \u0159e\u010di&scaron;ti. Pro sni\u017eov&aacute;n&iacute; mno\u017estv&iacute; bublinek se p\u0159ed pot&aacute;p\u011bn&iacute;m doporu\u010duj&iacute; n&aacute;sleduj&iacute;c&iacute; kroky: fyzick&eacute; cvi\u010den&iacute;, pobyt v teple, hydratace, d&yacute;ch&aacute;n&iacute; kysl&iacute;ku, vibrace aplikovan&eacute; na cel&eacute; t\u011blo a konzumace potravin obsahuj&iacute;c&iacute;ch antioxidanty.<br \/>C&eacute;vn&iacute; embolie (<strong>VGE<\/strong>) se dlouho pova\u017eovala za p\u0159&iacute;\u010dinu (i marker) DCS. &nbsp;\u010c&iacute;m v&iacute;ce bublinek v krvi, t&iacute;m pravd\u011bpodobn\u011bji se dostav&iacute; p\u0159&iacute;znaky dekomprese. A jak je uvedeno ji\u017e shora, bublinky se tvo\u0159&iacute; na hydrofobn&iacute;ch nuklea\u010dn&iacute;ch bodech na povrchu krevn&iacute;ch c&eacute;v. Z tohoto d\u016fvodu je z\u0159ejm&eacute;, \u017ee udr\u017eov&aacute;n&iacute; c&eacute;vn&iacute; soustavy (i cel&eacute;ho t\u011bla) v dobr&eacute; kondici m\u016f\u017ee v&eacute;st ke sn&iacute;\u017een&iacute; rizika DCS.<\/p>\n<p style=\"text-align: justify;\">V z&aacute;sad\u011b lze \u0159&iacute;ci, \u017ee fyzick&eacute; <strong>cvi\u010den&iacute;<\/strong> podporuje ventilaci a zbavov&aacute;n&iacute; se bublinek. Tepeln&yacute; stres m&aacute; za n&aacute;sledek produkci b&iacute;lkovin tepeln&eacute;ho &scaron;oku, kter&eacute; mohou tak&eacute; br&aacute;nit tvorb\u011b bublinek. Rovn\u011b\u017e dostate\u010dn&aacute; <strong>hydratace<\/strong> m&aacute; zn&aacute;m&eacute; preventivn&iacute; &uacute;\u010dinky. &Uacute;\u010dinn&aacute; hydratace se mus&iacute; prov&aacute;d\u011bt v\u017edy del&scaron;&iacute; dobu p\u0159ed ponorem, aby se voda dostala do t\u011bln&iacute;ch tk&aacute;n&iacute;. P&iacute;t hodn\u011b vody t\u011bsn\u011b p\u0159ed ponorem nepom\u016f\u017ee, t&iacute;m se pouze zv\u011bt&scaron;&iacute; objem tekutin v c&eacute;v&aacute;ch, co\u017e pak vede k rychl&eacute;mu &uacute;bytku tekutin b\u011bhem ponoru a to naopak nebezpe\u010d&iacute; DCS zvy&scaron;uje. Mechanick&aacute; <strong>vibrace<\/strong> aplikovan&aacute; na cel&eacute; t\u011blo p\u0159ed ponorem sni\u017euje nebezpe\u010d&iacute; DCS z\u0159ejm\u011b d&iacute;ky vyt\u011bsn\u011bn&iacute; ji\u017e existuj&iacute;c&iacute;ch mikrobublinek. Proto tak&eacute; n\u011bkte\u0159&iacute; provozovatel&eacute; pot&aacute;p\u011b\u010dsk&yacute;ch st\u0159edisek up\u0159ednost\u0148uj&iacute; p\u0159epravu na m&iacute;sto pot&aacute;p\u011bn&iacute; v rychl&eacute;m lehk&eacute;m motorov&eacute;m \u010dlunu. &nbsp;D\u016fle\u017eitou roli v eliminaci nuklea\u010dn&iacute;ch center bublinek m&aacute; lymfatick&aacute; dren&aacute;\u017e, kter&aacute; je rovn\u011b\u017e podporov&aacute;na vibracemi p\u0159ed ponorem. A kone\u010dn\u011b i d&yacute;ch&aacute;n&iacute; stoprocentn&iacute;ho <strong>kysl&iacute;ku&nbsp;<\/strong>p\u0159ed ponorem pat\u0159&iacute; k &uacute;\u010dinn&yacute;m zp\u016fsob\u016fm, jak dostat dus&iacute;k z t\u011bla ven.<\/p>\n<p style=\"text-align: justify;\">Rovn\u011b\u017e se zjistilo, \u017ee zat&iacute;mco lehk&eacute; fyzick&eacute; cvi\u010den&iacute; m&aacute; ochrann&eacute; &uacute;\u010dinky, <strong>n&aacute;ro\u010dn&eacute; cvi\u010den&iacute;&nbsp;<\/strong>ve spojen&iacute; s pot&aacute;p\u011bn&iacute;m lze pova\u017eovat z d\u016fvodu jeho proz&aacute;n\u011btliv&yacute;ch &uacute;\u010dink\u016f na krevn&iacute; c&eacute;vy za v&yacute;znamn&yacute; rizikov&yacute; faktor zvy&scaron;uj&iacute;c&iacute; nebezpe\u010d&iacute; rozvoje DCS. Dopady r\u016fzn\u011b intenz&iacute;vn&iacute;ch fyzick&yacute;ch cvi\u010den&iacute; prov&aacute;d\u011bn&yacute;ch p\u0159ed ponory, mezi ponory a po ponorech zkoumal Z. Dujic z L&eacute;ka\u0159sk&eacute; fakulty Univerzity Split v Chorvatsku. &nbsp;<br \/>Cvi\u010den&iacute; po pot&aacute;p\u011bn&iacute; m\u016f\u017ee vyvolat tzv. arterializaci \u017eil &ndash; otev\u0159en&iacute; drobn&yacute;ch otvor\u016f ve st\u011bn\u011b \u017e&iacute;ly, kter&eacute; umo\u017e\u0148uj&iacute;c&iacute; pr\u016fchod bublinek ze \u017eiln&iacute; do tepenn&eacute; krve &ndash; co\u017e je jev, kter&eacute;mu se tak&eacute; \u0159&iacute;k&aacute; <em>shunting<\/em> (p\u0159esunov&aacute;n&iacute;) a kter&yacute; je spojen se zv&yacute;&scaron;en&yacute;m nebezpe\u010d&iacute;m vzniku DCS. Intenzita arterializace je u ka\u017ed&eacute;ho jedince individu&aacute;ln&iacute;. U n\u011bkoho m\u016f\u017ee i prost&eacute; plav&aacute;n&iacute; na hladin\u011b, nebo p\u0159en&aacute;&scaron;en&iacute; v&yacute;stroje po ponoru vyvolat tak silnou arterializaci, \u017ee se p\u0159ekro\u010d&iacute; jej&iacute; mezn&iacute; hodnota. <br \/>Zjistilo se, \u017ee arterializaci p\u0159i cvi\u010den&iacute; se d&aacute; zabr&aacute;nit d&yacute;ch&aacute;n&iacute;m stoprocentn&iacute;ho kysl&iacute;ku. O<sub>2<\/sub> vyvol&aacute;v&aacute; smr&scaron;\u0165ov&aacute;n&iacute; c&eacute;v s n&aacute;sledn&yacute;m uzav\u0159en&iacute;m otvor\u016f (<em>shunt\u016f<\/em>).
