Velk&eacute; c&eacute;vy stejn\u011b jako nejmen&scaron;&iacute; c&eacute;vy, resp.kapil&aacute;rn&iacute; \u0159e\u010di&scaron;t\u011b a m&iacute;sta v&yacute;m\u011bny krevn&iacute;ch plyn\u016f jsou ve funkci po ponoru oslabeny a je&scaron;t\u011b v&yacute;razn\u011bji se toto projevu p\u0159i vzniku DCS. V&yacute;zkumy prov&aacute;d\u011bn&eacute; u n&aacute;dechov&yacute;ch pot&aacute;p\u011b\u010d\u016f ( u kter&yacute;ch se nevytv&aacute;\u0159&iacute; bubliny) nazna\u010duj&iacute;, \u017ee hydrostatick&yacute; tlak spolu s hyperoxi&iacute; (vy&scaron;&scaron;&iacute; hladinou kysl&iacute;ku) d&aacute;vaj&iacute; vzniknout oxidativn&iacute;mu stresu, kter&yacute; spou&scaron;t&iacute; destruktivn&iacute; efekt na endotel s n&aacute;sledkem odum\u0159en&iacute; bun\u011bk. N\u011bkter&eacute; z&aacute;va\u017en&eacute; &nbsp;p\u0159&iacute;pady DCS jsou v&scaron;ak bez po&scaron;kozen&iacute; endotelu a z toho se d&aacute; vyvodit, \u017ee kl&iacute;\u010dovou roli nehraje samotn&yacute; endotel, ale cirkuluj&iacute;c&iacute; faktory, kter&yacute; v n\u011bm vznikaj&iacute;. Jedn&iacute;m z nich &nbsp;je kysli\u010dn&iacute;k dusn&yacute; (NO), kter&yacute; je produkov&aacute;n endotelem. Blok&aacute;dou NO ve studi&iacute;ch na zv&iacute;\u0159atech v&yacute;zkumn&iacute;ci &nbsp;odhalili vztahy z&aacute;visl&eacute; na pohlav&iacute;: vznik DCS &nbsp;zap\u0159&iacute;\u010din\u011bn&yacute; touto blok&aacute;dou je \u010dast\u011bj&scaron;&iacute; u \u017een ale nikoliv &nbsp;u mu\u017e\u016f. T&iacute;mto sm\u011brem se do budoucna bude zab&yacute;vat v&yacute;zkum- tedy pohlavn&iacute;mi rozd&iacute;ly vzniku DCS mechanizm\u016f.<\/p>\n<p style=\"text-align: justify;\">Je zn&aacute;mo, \u017ee kysl&iacute;k je d\u016fle\u017eit&yacute; plyn v pot&aacute;p\u011bn&iacute; a \u017ee d&yacute;ch&aacute;n&iacute; &nbsp;\u010dist&eacute;ho kysl&iacute;ku je b\u011b\u017en&yacute;m a &uacute;\u010dinn&yacute;m l&eacute;kem v r&aacute;mci prvn&iacute; pomoci u DCS.Av&scaron;ak<strong>&nbsp;kysl&iacute;k m&aacute; tak&eacute; negativn&iacute;<\/strong> <strong>&uacute;\u010dinky.<\/strong>V&yacute;zkum v ter&eacute;nu byl prov&aacute;d\u011bn&yacute; J.Kotem (n&aacute;rodn&iacute; centrum pro hyperbarickou medic&iacute;nu, L&eacute;ka\u0159sk&aacute; fakulta Gda\u0148sk, Polsko).Oxidativn&iacute; stres je zp\u016fsoben destruktivn&iacute;m efektem voln&yacute;ch radik&aacute;l\u016f (O2,H2O2,OH), vytv&aacute;\u0159en&yacute;ch nekompletn&iacute; &nbsp;redukc&iacute; kysl&iacute;kov&yacute;ch vazeb v bu\u0148ce.Tyto radik&aacute;ly jsou &nbsp;agresivn&iacute;, kr&aacute;tce reaguj&iacute;c&iacute; molekuly,kter&eacute; po&scaron;kozuji DNA, proteinov&eacute; a lipidov&eacute; molekuly. Na&scaron;t\u011bst&iacute; &nbsp;je lidsk&eacute; t\u011blo vybaveno anitioxida\u010dn&iacute;m obrann&yacute;m syst&eacute;mem, kter&yacute; \u0159&iacute;d&iacute; &nbsp;vyv&aacute;\u017eenost mezi antioxidanty a radik&aacute;ly a t&iacute;m kontroluje rozsah po&scaron;kozen&iacute;. Nejvy&scaron;&scaron;&iacute; parci&aacute;ln&iacute; tlak je v plic&iacute;ch, kter&eacute; funguj&iacute; jako obrann&yacute; syst&eacute;m prvn&iacute; linie.Destruktivn&iacute; efekt m\u016f\u017ee v&eacute;st k fibroze (zmno\u017een&iacute; vaziva) plicn&iacute; tk&aacute;n\u011b. Kdy\u017e se &nbsp;toto stane, pacient trp&iacute; hypoxi&iacute;, i kdy\u017e d&yacute;ch&aacute; \u010dist&yacute; kysl&iacute;k. Relativn\u011b dobrou &nbsp;novou zpr&aacute;vou je, \u017ee tento typ oxidativn&iacute;ho stresu postihuje v&iacute;ce &nbsp;technick&eacute; pot&aacute;p\u011b\u010de u\u017e&iacute;vaj&iacute;c&iacute; sm\u011bsi s kysl&iacute;kem, ne\u017e na mnohem v\u011bt&scaron;&iacute; skupinu rekrea\u010dn&iacute;ch pot&aacute;p\u011b\u010d\u016f, kte\u0159&iacute; u\u017e&iacute;vaj&iacute; nap\u0159.Nitrox.<\/p>\n<p style=\"text-align: justify;\">V&yacute;zkumy programu PHYPODE ukazuj&iacute;, \u017ee existuje zjevn&aacute; individu&aacute;ln&iacute; vn&iacute;mavost k DCS, jak vypl&yacute;v&aacute; z n&aacute;lez\u016f skupin pot&aacute;p\u011b\u010d\u016f s tendenc&iacute; k rychl&eacute;mu vzniku c&eacute;vn&iacute;ch zkrat\u016f, pot&aacute;p\u011b\u010d\u016f s vysok&yacute;m v&yacute;skytem bublin po ponoru, pot&aacute;p\u011b\u010d\u016f s r\u016fznou kvalitou endotelu, genetick&yacute;mi dispozicemi a jin&yacute;mi. Jak je zm&iacute;n\u011bno v&yacute;&scaron;e, dobrou cestou ke sn&iacute;\u017een&iacute; v&yacute;skytu DCS je &nbsp;j&iacute; p\u0159edej&iacute;t. Dal&scaron;&iacute;mi parametry byla hloubka, &nbsp;doba ponoru, pozice pod vodou, frekvence pohyb\u016f ploutvemi, srde\u010dn&iacute; frekvence, sestupov&aacute; a v&yacute;stupov&aacute; rychlost pot&aacute;p\u011b\u010de.spolehlivost sensor\u016f byla testov&aacute;na &nbsp;a z&iacute;skan&eacute; hodnoty byly transformov&aacute;ny v pou\u017eiteln&aacute; data, kter&aacute; byla elektronicky nebo bezdr&aacute;tov\u011b p\u0159en&aacute;&scaron;ena do pam\u011b\u0165ov&eacute; jednotky s odpov&iacute;daj&iacute;c&iacute; &nbsp;velikost&iacute; pam\u011bti.<\/p>\n<p style=\"text-align: justify;\">Pot&aacute;p\u011b\u010dsk&aacute; medic&iacute;na se vyv&iacute;j&iacute; a jsou z&iacute;sk&aacute;v&aacute;na nov&aacute; data o mechanismu dekomprese. Dosud vzhledem k neodpov&iacute;daj&iacute;c&iacute; technologii sensor\u016f, byla fyziologick&aacute; data &nbsp;sb&iacute;r&aacute;na pouze v laborato\u0159i p\u0159ed a po ponoru, ale nikdy ne b\u011bhem ponoru. Arne Sieber (v\u011bdec v&yacute;zkumn&iacute;k v IMEGO AB, Gothenburg, &Scaron;v&eacute;dsko) rozvinul nov&eacute; senzorov&eacute; technologie k monitorov&aacute;n&iacute; <strong>EKG a t\u011blesn&eacute; teploty<\/strong> b\u011bhem ponoru.
