Scuba pot&aacute;p\u011b\u010di o n\u011bm v\u011bt&scaron;inou nikdy ani nesly&scaron;eli a bohu\u017eel je tento stav nezn&aacute;m&yacute; i v\u011bt&scaron;in\u011b l&eacute;ka\u0159\u016f. O tomto m&aacute;lo zn&aacute;m&eacute;m, &scaron;patn\u011b definovan&eacute;m a ne zcela pochopen&eacute;m jevu existuje \u0159ada b&aacute;chorek. Tak se do tohoto t&eacute;matu nyn&iacute; &bdquo;hluboce pono\u0159me&ldquo; a doufejme, \u017ee si t&iacute;m nep\u0159ivod&iacute;me nav&iacute;c je&scaron;t\u011b &bdquo;stla\u010den&iacute; mozku&ldquo;!<\/p>\n<p style=\"text-align: justify;\"><\/p>\n<p style=\"text-align: justify;\"><strong><u>Definice:<\/u><\/strong><\/p>\n<p style=\"text-align: justify;\">&bdquo;Stla\u010den&iacute; plic&ldquo; je n\u011bkdy tak&eacute; zn&aacute;m&eacute; pod pojmem stla\u010den&iacute; hrudn&iacute;ku nebo, form&aacute;ln\u011bji, pod n&aacute;zvem <em>sestupov&eacute; pulmon&aacute;ln&iacute; barotrauma (pulmonary barotrauma of descent,<span style=\"color: rgb(51,51,51);\">barotrauma plic z&nbsp;<\/span><span style=\"color: rgb(255,0,0);\"><span style=\"color: rgb(51,51,51);\">podtlaku<\/span> ,<\/span>ozna\u010dovan&eacute; zkratkou PBT, jak budeme tento stav naz&yacute;vat v cel&eacute;m \u010dl&aacute;nku).&nbsp;<\/em><\/p>\n<p style=\"text-align: justify;\">PBT tedy znamen&aacute;: <em>Poran\u011bn&iacute; nebo po&scaron;kozen&iacute; plic, ke kter&eacute;mu do&scaron;lo b\u011bhem pot&aacute;p\u011bn&iacute; se zadr\u017een&yacute;m dechem n&aacute;sledkem &uacute;\u010dink\u016f zv&yacute;&scaron;en&eacute;ho tlaku okoln&iacute;ho prost\u0159ed&iacute; na uzav\u0159en&eacute; plicn&iacute; prostory napln\u011bn&eacute; plynem. &nbsp;<\/em><\/p>\n<p style=\"text-align: justify;\">Tento popis a definice odkazuje na situaci, kter&aacute; nast&aacute;v&aacute; v plic&iacute;ch z d\u016fvodu zv&yacute;&scaron;en&eacute;ho okoln&iacute;ho tlaku b\u011bhem sestupu. N\u011bkte\u0159&iacute; odborn&iacute;ci pova\u017euj&iacute; za sou\u010d&aacute;st &scaron;ir&scaron;&iacute;ho pojet&iacute; PBT i stla\u010den&iacute; pr\u016fdu&scaron;nice.&nbsp;<\/p>\n<p style=\"text-align: justify;\">Za pou\u017eit&iacute; t&eacute;to nedokonal&eacute; definice uva\u017eujme nyn&iacute; o tom, jak ovliv\u0148uje zv&yacute;&scaron;en&yacute; okoln&iacute; tlak uzav\u0159en&eacute; plicn&iacute; prostory napln\u011bn&eacute; plynem.&nbsp;<\/p>\n<p style=\"text-align: justify;\"><\/p>\n<p style=\"text-align: justify;\"><strong><u>Fyzika a fyziologie:<\/u><\/strong><\/p>\n<p style=\"text-align: justify;\">Pro lep&scaron;&iacute; pochopen&iacute; PBT si mus&iacute;me zopakovat n\u011bco z fyziky a fyziologie.<\/p>\n<p style=\"text-align: justify;\">Za prv&eacute; &ndash; Boyl\u016fv z&aacute;kon:<em>&nbsp;&bdquo;Objem plynu je za st&aacute;l&eacute; teploty nep\u0159&iacute;mo &uacute;m\u011brn&yacute; jeho tlaku.&ldquo; &nbsp;<\/em><\/p>\n<p style=\"text-align: justify;\">Podle tohoto z&aacute;kona se objem plynu v uzav\u0159en&eacute;m syst&eacute;mu sni\u017euje, jestli\u017ee se zvy&scaron;uje okoln&iacute; tlak a naopak. P\u0159i pot&aacute;p\u011bn&iacute; se zadr\u017een&yacute;m dechem pl&iacute;ce p\u0159edstavuj&iacute; uzav\u0159en&yacute; syst&eacute;m napln\u011bn&yacute; plynem, p\u0159i\u010dem\u017e se p\u0159i sestupu pod vodu zvy&scaron;uje okoln&iacute; tlak zp\u016fsoben&yacute; hydrostatick&yacute;m tlakem okoln&iacute; vody. Na ka\u017ed&yacute;ch 10 metr\u016f mo\u0159sk&eacute; vody (metres of seawater, MSW) se zvy&scaron;uje tlak o 1 atmosf&eacute;ru (ATM). Kdy\u017e se obsah plic vypr&aacute;zdn&iacute; na absolutn&iacute; minimum, z\u016fstane v plic&iacute;ch tzv. zbytkov&yacute; (rezidu&aacute;ln&iacute;) objem (RV). Jin&yacute;mi slovy, pot&aacute;p\u011bn&iacute; do hloubek se zadr\u017een&yacute;m dechem by m\u011blo b&yacute;t bezpe\u010dn&eacute; jen potud, dokud stla\u010den&iacute; (komprese) plic nezp\u016fsob&iacute; v\u011bt&scaron;&iacute; sn&iacute;\u017een&iacute; jejich objemu, ne\u017e \u010din&iacute; zbytkov&yacute; objem. RV p\u0159edstavuje obvykle 20 a\u017e 25% celkov&eacute;ho objemu plic po pln&eacute;m n&aacute;dechu. Podle Boyleova z&aacute;kona by se dos&aacute;hlo RV p\u0159ibli\u017en\u011b v hloubce 35 a\u017e 45 metr\u016f mo\u0159sk&eacute; vody (tj. p\u0159i tlaku 4,5 a\u017e 5,5 atmosf&eacute;ry), co\u017e by m\u011bla b&yacute;t mezn&iacute; hranice pot&aacute;p\u011bn&iacute; se zadr\u017een&yacute;m dechem. Tak tomu v&scaron;ak nen&iacute;, nebo\u0165 v sou\u010dasn&eacute; dob\u011b hloubkov&eacute; rekordy p\u0159i voln&eacute;m pot&aacute;p\u011bn&iacute; p\u0159ekra\u010duj&iacute; hodnoty 200 metr\u016f mo\u0159sk&eacute; vody. Tato skute\u010dnost napomohla k objevu dal&scaron;&iacute;ch fyziologick&yacute;ch mechanism\u016f, kter&eacute; zde hraj&iacute; rovn\u011b\u017e d\u016fle\u017eitou roli. Krom\u011b \u010d&aacute;ste\u010dn&eacute;ho kolapsu samotn&eacute; hrudn&iacute; dutiny pat\u0159&iacute; v t\u011bchto situac&iacute;ch k dal&scaron;&iacute;m d\u016fle\u017eit&yacute;m mechanism\u016fm i hromad\u011bn&iacute; krve z okoln&iacute;ch tk&aacute;n&iacute; v hrudi. Tento mechanismus umo\u017e\u0148uje akumulaci a\u017e 1,5 litru krve v krevn&iacute;ch c&eacute;v&aacute;ch uvnit\u0159 hrudn&iacute;ku.<\/p>\n<p style=\"text-align: justify;\">V z&aacute;sad\u011b plat&iacute;, \u017ee hromad\u011bn&iacute;m krve v hrudn&iacute;ku se vyrovn&aacute;v&aacute; dal&scaron;&iacute; n&aacute;r\u016fst tlaku pot&eacute;, kdy bylo dosa\u017eeno RV, co\u017e vlastn\u011b RV sni\u017euje a umo\u017e\u0148uje bezpe\u010dn&eacute; ponory do v\u011bt&scaron;&iacute;ch hloubek. Tento mechanismus v&scaron;ak zvy&scaron;uje tlak v plicn&iacute;m \u0159e\u010di&scaron;ti i v plicn&iacute;ch vl&aacute;se\u010dnic&iacute;ch s rizikem n&aacute;sledn&eacute;ho protr\u017een&iacute; a krv&aacute;cen&iacute;.<\/p>\n<p style=\"text-align: justify;\">V praxi tyto mechanismy umo\u017e\u0148uj&iacute;, aby se pl&iacute;ce &scaron;pi\u010dkov\u011b tr&eacute;novan&yacute;ch pot&aacute;p\u011b\u010d\u016f stla\u010dily a\u017e na 5% celkov&eacute; kapacity. A t&iacute;m se nab&iacute;z&iacute; i ot&aacute;zka, zdali tato hodnota (tj. 5% celkov&eacute; kapacity plic) ji\u017e znamen&aacute; absolutn&iacute; hranici pro sportovce praktikuj&iacute;c&iacute; pot&aacute;p\u011bn&iacute; se zadr\u017een&yacute;m dechem. Nen&iacute; zvl&aacute;&scaron;tnost&iacute;, \u017ee tito sportovci po vyno\u0159en&iacute; vyka&scaron;l&aacute;vaj&iacute; krev a jejich v&yacute;kon\u016fm se p\u0159ibl&iacute;\u017e&iacute; jen m&aacute;lokdo!<\/p>\n<p style=\"text-align: justify;\"><\/p>\n<p style=\"text-align: justify;\"><strong><u>P\u0159&iacute;znaky a projevy:<\/u><\/strong><\/p>\n<p style=\"text-align: justify;\">I kdy\u017e se m\u016f\u017ee zd&aacute;t, \u017ee PBT se t&yacute;k&aacute; pouze ponor\u016f do velk&yacute;ch hloubek, existuj&iacute; i zpr&aacute;vy popisuj&iacute;c&iacute; v&yacute;skyt PBT p\u0159i ponorech do m&iacute;rn&yacute;ch hloubek &ndash; zpravidla p\u0159i opakovan&yacute;ch ponorech s kr&aacute;tk&yacute;mi p\u0159est&aacute;vkami na hladin\u011b, p\u0159i\u010dem\u017e sta\u010d&iacute;, aby se takov&eacute; ponory uskute\u010dnily do hloubky pouh&yacute;ch 4 metr\u016f mo\u0159sk&eacute; vody. Mo\u017en&aacute; zde hraje roli krom\u011b komprese v&iacute;ce faktor\u016f. Je dob\u0159e zn&aacute;mo, \u017ee v plic&iacute;ch se m\u016f\u017ee shroma\u017e\u010fovat tekutina tak&eacute; pobytem t\u011bla ve vod\u011b. Sta\u010d&iacute; pono\u0159en&iacute; t\u011bla pod vodu s hlavou nad hladinou, nebo plav&aacute;n&iacute; na hladin\u011b. P\u0159i rozvoji PBT hraj&iacute; u ka\u017ed&eacute;ho jednotlivce d\u016fle\u017eitou roli tak&eacute; anatomick&eacute;, fyziologick&eacute; i patologick&eacute; p\u0159edpoklady, kter&eacute; se nav&iacute;c mohou den ode dne m&iacute;rn\u011b m\u011bnit.<\/p>\n<p style=\"text-align: justify;\">Rozhodn\u011b ne v&scaron;echny p\u0159&iacute;pady PBT jsou rozpozn&aacute;ny. N\u011bkter&eacute; p\u0159&iacute;znaky jsou jen p\u0159echodn&eacute;, dal&scaron;&iacute; se mohou zam\u011bnit s p\u0159&iacute;znaky typick&yacute;mi pro jin&aacute; onemocn\u011bn&iacute;, jak&yacute;mi jsou ch\u0159ipka nebo z&aacute;pal plic. O PBT se jedn&aacute;, jestli\u017ee se dostav&iacute; n&aacute;sleduj&iacute;c&iacute; p\u0159&iacute;znaky (tj. pot&iacute;\u017ee) a projevy (tj. skute\u010dn\u011b prokazateln&eacute; abnormality):<\/p>\n<p style=\"text-align: justify;\"><u>P\u0159&iacute;znaky:<\/u> bolesti hrudn&iacute;ku, kr&aacute;tk&yacute; dech, pocit vody v plic&iacute;ch, ka&scaron;el, &uacute;nava, pocit stla\u010den&iacute; nebo sv&iacute;r&aacute;n&iacute; hrudn&iacute;ku p\u0159i sestupu pod vodu, z&aacute;vra\u0165, nucen&iacute; ke zvracen&iacute; (nauzea), slabost, brn\u011bn&iacute; a zm\u011bny citlivosti k\u016f\u017ee (parest&eacute;zie), mal&aacute;tnost.<\/p>\n<p style=\"text-align: justify;\"><u>Projevy:<\/u> zrychlen&eacute; d&yacute;ch&aacute;n&iacute;, vyka&scaron;l&aacute;v&aacute;n&iacute; jasn\u011b \u010derven&eacute; krve, vyka&scaron;l&aacute;v&aacute;n&iacute; zp\u011bn\u011bn&eacute; krve, zvracen&iacute;, dechov&aacute; t&iacute;se\u0148, ztr&aacute;ta orientace, ztr&aacute;ta v\u011bdom&iacute;, neurologick&eacute; v&yacute;padky, kardiorespira\u010dn&iacute; (srde\u010dn&iacute;) z&aacute;stava, smrt.<\/p>\n<p style=\"text-align: justify;\">Z dlouh&eacute;ho seznamu p\u0159&iacute;znak\u016f a projev\u016f je patrn&eacute;, \u017ee PBT m\u016f\u017ee dosahovat intenzity od m&iacute;rn&eacute;ho podr&aacute;\u017ed\u011bn&iacute; a\u017e po stav kon\u010d&iacute;c&iacute; smrt&iacute;. Co se t&yacute;\u010de d&eacute;lky, m\u016f\u017ee se jednat o velmi kr&aacute;tkou p\u0159&iacute;hodu, ale i n\u011bkolikam\u011bs&iacute;\u010dn&iacute; probl&eacute;m.
