Dysfunkce Eustachovy trubice (ETD) je b\u011b\u017ený klinický problém st\u0159edního ucha, který postihuje pacienty všech v\u011bkových kategorií. P\u0159í\u010diny ETD jsou vn\u011bjší i vnit\u0159ní, \u010dasto souvisejí s nosem, nosohltanem nebo se zán\u011btem \u010di chorobným zú\u017eením samotné Eustachovy trubice. K p\u0159íznak\u016fm pat\u0159í ztráta sluchu, „tupé“ slyšení a n\u011bkdy (velmi výjime\u010dn\u011b) i bolest ucha. Tyto p\u0159íznaky se mohou dostavit po r\u016fzných \u010dinnostech: cestování letadlem nebo velmi rychlým vlakem, horské turistice, ale hlavn\u011b b\u011bhem potáp\u011bní, a to jak p\u0159i volném potáp\u011bní, tak i p\u0159i potáp\u011bní p\u0159ístrojovém.<\/p>\n

Tento velmi d\u016fle\u017eitý kanálek, který spojuje st\u0159ední ucho (bubínkovou dutinu a vedlejší dutiny) s nosohltanem, byl známý ji\u017e v 16. století. Eustachova trubice je tvo\u0159ena kostnatou \u010dástí a fibrokartilaginózní (vazivovou) \u010dástí. Její funkce spo\u010dívá ve ventilaci, drená\u017ei a ochran\u011b st\u0159edního ucha, jako\u017e i v udr\u017eování takového tlaku v bubínkové dutin\u011b, aby odpovídal tlaku existujícímu ve vn\u011bjším prost\u0159edí (atmosférickému tlaku). Jen díky této vyrovnávací funkci umo\u017en\u011bné Eustachovou trubicí m\u016f\u017ee systém sestávající z ušního bubínku a sluchových k\u016fstek fungovat správným zp\u016fsobem, navíc tento kanálek zabra\u0148uje hromad\u011bní tekutých nebo katarálních sekrecí v bubínkové dutin\u011b.<\/p>\n

<\/p>\n

ETD vyvolává problémy s ventilací ve vnit\u0159ním uchu, co\u017e mívá za následek nedokonalé vyrovnávání tlaku mezi vnit\u0159ním uchem a vn\u011bjším prost\u0159edím, jako\u017e i tvorbu a setrvávání sekrecí v bubínkové dutin\u011b. To m\u016f\u017ee vyvrcholit silným barotraumatem st\u0159edního ucha, tedy stavem, kterého se potáp\u011b\u010di tak obávají.<\/p>\n

Jestli\u017ee dojde k ETD b\u011bhem p\u0159ístrojového potáp\u011bní, nap\u0159. z d\u016fvodu rychlého nár\u016fstu okolního tlak p\u0159i sestupu, není mo\u017ené spontánn\u011b vyrovnávat tlak v dutinách st\u0159edního ucha, proto\u017ee fyziologickému otev\u0159ení Eustachovy trubice brání krátkodobý negativní bubínkový tlak, který udr\u017euje st\u011bny trubice „vmá\u010dknuté“.<\/p>\n

Kdy\u017e nefunguje normální vyrovnávání tlaku, m\u016f\u017ee se potáp\u011b\u010d pokusit provést n\u011bjaký speciální vyrovnávací manévr. Základem pro ú\u010dinné a bezrizikové vyrovnávání tlaku ve st\u0159edním uchu p\u0159i sestupu je v\u010dasné zahájení a správné provád\u011bní vyrovnávacích manévr\u016f, jinými slovy – otvírání Eustachovy trubice pomocí malých „tlakových posun\u016f“ (ji\u017e od prvních metr\u016f sestupu).<\/p>\n

Diagnózu ETD lze stanovit odborným vyšet\u0159ením ORL, ale také p\u0159ístrojovým vyšet\u0159ením funk\u010dnosti sluchu a kladeného odporu. Tímto zp\u016fsobem lze také objasnit p\u0159í\u010diny dysfunkce: p\u0159ítomnost adenoidní vegetace nebo novotvaru v nosohltanu, deformace nosní p\u0159epá\u017eky, hypertrofie nosních sko\u0159ep, zán\u011bt Eustachovy trubice atd.<\/p>\n

Navíc k b\u011b\u017en\u011b pou\u017eívaným terapiím (termální intratympanické profukování, kinesioterapie pou\u017eitím balónku Otovent, pou\u017eití Ear Popper) se dnes rovn\u011b\u017e pou\u017eívá chirurgická metoda, která tento klinický problém m\u016f\u017ee také vylé\u010dit – balónková tuboplastika.  <\/p>\n

P\u0159i tomto postupu se pou\u017eije upravený katétr PTA, který se zavede do Eustachovy trubice za pou\u017eití speciálního mikroendoskopu, který umo\u017ení jeho jemné a p\u0159esné polohování. Jakmile je katétr zaveden, nafoukne se solným roztokem balónek umíst\u011bný na jeho distálním konci, a sice a\u017e na tlak 10 bar. Tento tlak se udr\u017euje po dobu dvou minut. Potom se balónek vypustí a katétr se opatrn\u011b (op\u011bt pod endoskopickou kontrolou) vytáhne.<\/p>\n

Pooperativní výsledky se vyhodnocují na speciální stupnici ETS, kde jsou uvedeny rozsahy objektivních a subjektivních parametr\u016f.<\/p>\n

Výsledky této metody prokazují, \u017ee dilatace Eustachovy trubice je bezpe\u010dný a jednoduchý postup, který významn\u011b usnad\u0148uje lé\u010dbu stenózy (chorobného zú\u017eení) ušní trubice.<\/p>\n","protected":false},"excerpt":{"rendered":"
