Pravdou je, \u017ee sportovn&iacute; pot&aacute;p\u011b\u010di mnohem pravd\u011bpodobn\u011bji za\u017eij&iacute; IGN ne\u017e dekompresn&iacute; onemocn\u011bn&iacute; (DCI). I kdy\u017e se prok&aacute;zalo, \u017ee pot&aacute;p\u011b\u010di si na nark&oacute;zu vypracovat toleranci nemohou, p\u0159esto je mo\u017en&eacute; nau\u010dit se, jak se d&aacute; vypo\u0159&aacute;dat s r\u016fzn&yacute;mi stupni tohoto nebezpe\u010dn&eacute;ho stavu.<\/p>\n<p style=\"text-align: justify;\">V ran&yacute;ch dob&aacute;ch p\u0159&iacute;strojov&eacute;ho pot&aacute;p\u011bn&iacute; byla dus&iacute;kov&aacute; nark&oacute;za hlavn&iacute;m omezuj&iacute;c&iacute;m faktorem p\u0159i ponorech pod 30-40 metr\u016f, p\u0159i\u010dem\u017e pot&aacute;p\u011b\u010di museli \u0159e&scaron;it sv\u016fj stav na vlastn&iacute; nebezpe\u010d&iacute;, kdy\u017e se odv&aacute;\u017eili sestoupit hloub\u011bji. Dnes, d&iacute;ky mo\u017enostem pou\u017e&iacute;vat plynn&eacute; sm\u011bsi, lze sn&iacute;\u017eit riziko plynouc&iacute; z IGN pom\u011brn\u011b snadno.<\/p>\n<p style=\"text-align: justify;\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" src=\"\/wp-content\/uploads\/imgimport\/1013\/divers3-1500.jpg\" style=\"width: auto;\"><\/p>\n<p style=\"text-align: justify;\">P\u0159esto v&scaron;ak st&aacute;le p\u0159etrv&aacute;v&aacute; diskuse, jak&yacute; rozsah nark&oacute;zy je je&scaron;t\u011b &bdquo;p\u0159ijateln&yacute;&ldquo;, do jak&eacute; m&iacute;ry se d&aacute; nark&oacute;za zvl&aacute;dnout a zdali i kysl&iacute;k p\u0159isp&iacute;v&aacute; k narkotick&eacute; &uacute;\u010dinnosti pot&aacute;p\u011b\u010dova d&yacute;chac&iacute;ho plynu. V sou\u010dasnosti plat&iacute;, \u017ee <strong><a href=\"https:\/\/alertdiver.eu\/cs_CZ\/clanky\/poblouzneni-technikou-hloubka-narkoza-a-skolici-agentury\/\" target=\"_blank\" rel=\"noopener\">praxe existuj&iacute;c&iacute; v komunit\u011b sportovn&iacute;ch pot&aacute;p\u011b\u010d\u016f se velice r\u016fzn&iacute;<\/a><\/strong>.<\/p>\n<p style=\"text-align: justify;\">Hlavn&iacute;m probl&eacute;mem p\u0159i studiu IGN je neexistence objektivn&iacute;ho a spolehliv&eacute;ho zji&scaron;\u0165ov&aacute;n&iacute; n&aacute;stupu a z&aacute;va\u017enosti (rozsahu) zm&iacute;n\u011bn&eacute; nark&oacute;zy. Pot&aacute;p\u011b\u010di se uk&aacute;zali jako nespolehliv&eacute; zdroje informac&iacute;, co se t&yacute;\u010de jejich vlastn&iacute;ho hodnocen&iacute; subjektivn&iacute;ch p\u0159&iacute;znak\u016f a tradi\u010dn&iacute; psychologick&eacute; testov&aacute;n&iacute; se prov&aacute;d&iacute; pod vodou jen velmi obt&iacute;\u017en\u011b.<\/p>\n<p style=\"text-align: justify;\">V minul&eacute;m desetilet&iacute; v&scaron;ak v&yacute;zkumn&iacute; pracovn&iacute;ci z DAN Europe zve\u0159ejnili n\u011bkolik studi&iacute; obsahuj&iacute;c&iacute;ch hodnocen&iacute; &uacute;\u010dinnosti nov&eacute;ho n&aacute;stroje pro posouzen&iacute; pot&aacute;p\u011b\u010dov&yacute;ch kognitivn&iacute;ch schopnost&iacute;. Tato metoda se naz&yacute;v&aacute; <em>kritick&aacute; frekvence blik&aacute;n&iacute;-f&uacute;ze<\/em> (CFFF) a zd&aacute; se b&yacute;t objektivn&iacute;m m\u011b\u0159&iacute;tkem IGN, kter&eacute; lze snadno pou\u017e&iacute;vat.<\/p>\n<p style=\"text-align: justify;\">Tento \u010dl&aacute;nek se zam\u011b\u0159&iacute; na dv\u011b studie nab&iacute;zej&iacute;c&iacute; nov&eacute; a doslova p\u0159ekvapiv&eacute; porozum\u011bn&iacute; a pochopen&iacute; IGN. V prvn&iacute; studii, jej&iacute;\u017e v&yacute;sledky byly zve\u0159ejn\u011bny v roce 2016, se pod n&aacute;zvem <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27737486\" target=\"_blank\" rel=\"noopener\">&ldquo;Do Environmental Conditions Contribute to Narcosis Onset and Symptom Severity?<\/a>&rdquo; zji&scaron;\u0165ovaly &uacute;\u010dinky r\u016fzn&yacute;ch hyperbarick&yacute;ch prost\u0159ed&iacute; na IGN. Do&scaron;lo se k z&aacute;v\u011bru, \u017ee tlak a plyn mohou b&yacute;t jedin&yacute;mi extern&iacute;mi faktory ovliv\u0148uj&iacute;c&iacute;mi nark&oacute;zu. Tak&eacute; se zjistilo, \u017ee n&aacute;stup IGN za\u010d&iacute;n&aacute; po kr&aacute;tk&eacute; f&aacute;zi zv&yacute;&scaron;en&eacute; ment&aacute;ln&iacute; ostrosti bezprost\u0159edn\u011b po sestupu a &bdquo;dozvuky&ldquo; nark&oacute;zy p\u0159etrv&aacute;vaj&iacute; alespo\u0148 30 minut po ponoru.<\/p>\n<p style=\"text-align: justify;\">V druh&eacute; studii nazvan&eacute; &ldquo;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/31177518\" target=\"_blank\" rel=\"noopener\">Early detection of diving-related cognitive impairment of different nitrogen-oxygen gas mixtures using critical flicker fusion frequency<\/a>,&ldquo; kdy se srovn&aacute;vala dus&iacute;kov&aacute; nark&oacute;za p\u0159i ponorech se vzduchem a s nitroxem (EANx) pomoc&iacute; jak CFFF, tak i pou\u017eit&iacute;m tradi\u010dn&iacute;ch psychologick&yacute;ch test\u016f, se do&scaron;lo k z&aacute;v\u011br\u016fm, \u017ee zv&yacute;&scaron;en&eacute; parci&aacute;ln&iacute; tlaky kysl&iacute;ku v EAN mohou b&yacute;t v&yacute;znamn&yacute;m modul&aacute;torem &uacute;\u010dink\u016f IGN.<\/p>\n<h3 style=\"\">Stru\u010dn\u011b k blik&aacute;n&iacute;-f&uacute;zi<\/h3>\n<p style=\"text-align: justify;\">Kritick&aacute; frekvence blik&aacute;n&iacute;-f&uacute;ze (CFFF) je frekvence, p\u0159i kter&eacute; se blikaj&iacute;c&iacute; sv\u011btlo vn&iacute;m&aacute; jako st&aacute;l&eacute; a nep\u0159etr\u017eit&eacute;. Tato metoda se vyvinula po\u010d&aacute;tkem dvac&aacute;t&eacute;ho stolet&iacute; a za\u010dala se pou\u017e&iacute;vat p\u0159i v&yacute;zkumu fyziologie vid\u011bn&iacute;. CFFF se stala d\u016fle\u017eit&yacute;m n&aacute;strojem pro m\u011b\u0159en&iacute; ment&aacute;ln&iacute; bd\u011blosti a ostrosti za podm&iacute;nek zahrnuj&iacute;c&iacute;ch patalogii, anestezi a pracovn&iacute; z&aacute;t\u011b\u017e p\u0159i l&eacute;t&aacute;n&iacute;.<\/p>\n<p style=\"text-align: justify;\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" src=\"\/wp-content\/uploads\/imgimport\/1013\/divers-1500.jpg\" style=\"width: auto;\"><\/p>\n<p style=\"text-align: justify;\">Kdy\u017e se kognitivn&iacute; funkce lid&iacute; zhor&scaron;uj&iacute; nebo sni\u017euj&iacute;, frekvence, p\u0159i kter&eacute; se vn&iacute;m&aacute; blik&aacute;n&iacute;, ust&aacute;v&aacute;, tj. &bdquo;f&uacute;zn&iacute; frekvence&ldquo; kles&aacute;. Naopak, p\u0159i stavech zv&yacute;&scaron;en&eacute; ment&aacute;ln&iacute; bd\u011blosti f&uacute;zn&iacute; frekvence stoup&aacute;. A jeliko\u017e r\u016fzn&iacute; jedinci vn&iacute;maj&iacute; odli&scaron;n&eacute; frekvence, jako z&aacute;kladn&iacute; f&uacute;zn&iacute; frekvence \u010dlov\u011bka se pova\u017euje 100% a CFFF se m\u011b\u0159&iacute; jako procento tohoto z&aacute;kladu.<\/p>\n<p style=\"text-align: justify;\">V roce 1960 poprv&eacute; upozornil zakladatel DAN USA, dr. Peter Bennett, na korelaci mezi ment&aacute;ln&iacute;mi stavy pot&aacute;p\u011b\u010d\u016f, CFFF a elektroencefalogramem (EEG) a p\u0159i n&aacute;sledn&yacute;ch v&yacute;zkumech se zjistilo, \u017ee zm\u011bny CFFF b\u011bhem ponoru s helioxovou saturac&iacute; se p\u0159ibli\u017en\u011b shoduj&iacute; se zm\u011bnami EEG. Dal&scaron;&iacute; v&yacute;zkumy v&scaron;ak tyto v&yacute;sledky nezopakovaly a od pou\u017e&iacute;v&aacute;n&iacute; CFFF se upustilo.&nbsp;<\/p>\n<p style=\"text-align: justify;\">V posledn&iacute;ch letech v&scaron;ak v&yacute;zkumn&iacute; pracovn&iacute;ci DAN tuto zd&aacute;nliv\u011b obskurn&iacute; metodu vzk\u0159&iacute;sili a zaznamenali s n&iacute; slibn&eacute; v&yacute;sledky. V roce 2012 toti\u017e prok&aacute;zali, \u017ee testy CFFF umo\u017e\u0148uj&iacute; <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22476770\" target=\"_blank\" rel=\"noopener\">spolehliv&eacute; m\u011b\u0159en&iacute; pod vodou<\/a> a poskytuj&iacute; v&yacute;sledky zji&scaron;\u0165ov&aacute;n&iacute; kognitivn&iacute;ch funkc&iacute; pot&aacute;p\u011b\u010de podobn&eacute;, jak&yacute;ch se dosahuje u test\u016f p\u0159i pou\u017eit&iacute; <a href=\"https:\/\/en.wikipedia.org\/wiki\/PEBL_(software)\" target=\"_blank\" rel=\"noopener\">psychologick&eacute;ho experiment&aacute;ln&iacute;ho jazyka&nbsp;<\/a>(PEBL), <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24122188\" target=\"_blank\" rel=\"noopener\">kdy\u017e se d&yacute;ch&aacute; vzduch a kysl&iacute;k za atmosf&eacute;rick&eacute;ho tlaku&nbsp;<\/a>(2014).<\/p>\n<p style=\"text-align: justify;\">Testy CFFF se prov&aacute;d\u011bj&iacute; pod vodou mnohem snadn\u011bji ne\u017e testy s PEBL a tak&eacute; je testovan&yacute; subjekt nem\u016f\u017ee nijak snadno ovlivnit. Uskute\u010d\u0148uj&iacute; se pomoc&iacute; mal&eacute;ho v&aacute;lce opat\u0159en&eacute;ho modrou kontrolkou LED a rotuj&iacute;c&iacute;m krou\u017ekem, kter&yacute; m\u011bn&iacute; frekvenci blik&aacute;n&iacute;. B\u011bhem testu se d&iacute;v&aacute; pot&aacute;p\u011b\u010d p\u0159&iacute;mo na kontrolku LED. Testuj&iacute;c&iacute; badatel pak frekvenci blik&aacute;n&iacute; zvy&scaron;uje nebo sni\u017euje. Kdy\u017e se pot&aacute;p\u011b\u010di za\u010dne jevit blik&aacute;n&iacute; kontrolky LED jako st&aacute;l&yacute; stav (f&uacute;ze), test se zastav&iacute; a f&uacute;zn&iacute; frekvence se zaznamen&aacute;. V&yacute;sledky se pot&eacute; porovnaj&iacute; s CFFF p\u0159&iacute;slu&scaron;n&eacute;ho jedince p\u0159ed ponorem, co\u017e slou\u017e&iacute; jako z&aacute;klad. V dan&eacute;m p\u0159&iacute;pad\u011b se zv&yacute;&scaron;en&iacute; f&uacute;zn&iacute; frekvence rovn&aacute; zv&yacute;&scaron;en&eacute; kognitivn&iacute; funkci a sn&iacute;\u017een&iacute; se pova\u017euje za d\u016fkaz existuj&iacute;c&iacute; nark&oacute;zy IGN.<\/p>\n<p style=\"text-align: justify;\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" src=\"\/wp-content\/uploads\/imgimport\/1013\/graphiquecolle-device-1000.jpg\" style=\"width: auto;\"><\/p>\n<h3 style=\"\">Jak dalece z&aacute;le\u017e&iacute; na prost\u0159ed&iacute;?<\/h3>\n<p style=\"text-align: justify;\">Jeliko\u017e se v&iacute;, \u017ee pot&aacute;p\u011b\u010di se pod vodou adaptuj&iacute;, v&scaron;eobecn\u011b se uzn&aacute;v&aacute;, \u017ee i samotn&eacute; prost\u0159ed&iacute; ovliv\u0148uje v&yacute;konnost, a to kombinac&iacute; r\u016fzn&yacute;ch faktor\u016f. Z tohoto d\u016fvodu se sleduje cel&aacute; \u0159ada faktor\u016f, kter&eacute; z\u0159ejm\u011b p\u0159isp&iacute;vaj&iacute; k n&aacute;stupu nark&oacute;zy IGN i k jej&iacute; intenzit\u011b.<\/p>\n<p style=\"text-align: justify;\">Za jeden z hlavn&iacute;ch faktor\u016f ovliv\u0148uj&iacute;c&iacute;ch IGN se pova\u017euj&iacute; zv&yacute;&scaron;en&eacute; parci&aacute;ln&iacute; tlaky oxidu uhli\u010dit&eacute;ho (PCO<sub>2<\/sub>) vznikl&eacute; vy\u010derp&aacute;n&iacute;m p\u0159i pr&aacute;ci, usilovn&yacute;m plav&aacute;n&iacute;m, ale tak&eacute; n&aacute;mahou p\u0159i samotn&eacute;m d&yacute;ch&aacute;n&iacute;. Zv&yacute;&scaron;en&eacute; parci&aacute;ln&iacute; tlaky PCO<sub>2<\/sub> toti\u017e vedou k roz&scaron;&iacute;\u0159en&iacute; mozkov&yacute;ch c&eacute;v, co\u017e m&aacute; za n&aacute;sledek vy&scaron;&scaron;&iacute; hladiny dus&iacute;ku v mozku. Dal&scaron;&iacute;m pravd\u011bpodobn&yacute;m faktorem je chlad, nebo\u0165 ten zp\u016fsobuje z&uacute;\u017een&iacute; perifern&iacute;ch c&eacute;v, ale proto\u017ee mozkov&eacute; c&eacute;vy se z&uacute;\u017eit nemohou, v&yacute;sledkem je zv&yacute;&scaron;en&eacute; zat&iacute;\u017een&iacute; mozku dus&iacute;kem. &nbsp;<\/p>\n<p style=\"text-align: justify;\">Dal&scaron;&iacute;mi faktory, kter&eacute; z\u0159ejm\u011b p\u0159isp&iacute;vaj&iacute; k nark&oacute;ze IGN, je u\u017eit&iacute; alkoholu nebo drog, kocovina nebo &uacute;nava, &uacute;zkost, p\u0159et&iacute;\u017eenost, stres, omezen&aacute; viditelnost, rychlost sestupu, z&aacute;vrat\u011b a prostorov&aacute; dezorientace. Z v\u011bdeck&eacute;ho pohledu v&scaron;ak pro tyto faktory neexistuje dostatek d\u016fkaz\u016f, proto je nutn&eacute; z&iacute;skat v tomto sm\u011bru v&iacute;ce p\u0159esn&yacute;ch &uacute;daj\u016f. Z tohoto d\u016fvodu se tato prvn&iacute; studie zam\u011b\u0159ila na pot&aacute;p\u011b\u010dsk&eacute; prost\u0159ed&iacute;.<\/p>\n<p style=\"text-align: justify;\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" src=\"\/wp-content\/uploads\/imgimport\/1013\/divers2-1500.jpg\" style=\"width: auto;\"><\/p>\n<p style=\"text-align: justify;\">V&yacute;zkumn&iacute;ci z&iacute;skali vyrovnanou skupinu slo\u017eenou ze 40 pot&aacute;p\u011b\u010d\u016f neku\u0159&aacute;k\u016f ve v\u011bku 30-40 let, kte\u0159&iacute; pravideln\u011b cvi\u010d&iacute; a jejich\u017e index t\u011blesn&eacute; hmotnosti (BMI) dosahoval hodnoty 20-25. Tito pot&aacute;p\u011b\u010di se 72 hodin p\u0159ed uskute\u010dn\u011bn&iacute;m v&yacute;zkumu zdr\u017eeli po\u017eit&iacute; jak&eacute;hokoli alkoholu.<\/p>\n<p style=\"text-align: justify;\">Ponory proveden&eacute; v r&aacute;mci testu byly do hloubky 30 metr\u016f po dobu 20 minut v nejni\u017e&scaron;&iacute;m bod\u011b, ale v t\u0159ech odli&scaron;n&yacute;ch prost\u0159ed&iacute;ch: v hyperbarick&eacute; komo\u0159e, v hlubok&eacute;m baz&eacute;nu Nemo 33 a na oce&aacute;nu. Teplota vody v baz&eacute;nu byla 33&deg;C, neexistovala \u017e&aacute;dn&aacute; tepeln&aacute; ochrana. Teplota oce&aacute;nu byla 19&deg;C a pot&aacute;p\u011b\u010di m\u011bli na sob\u011b \u0159&aacute;dn&eacute; neopr&eacute;nov&eacute; kombin&eacute;zy.<\/p>\n<p>U ka\u017ed&eacute;ho ponoru se provedlo p\u011bt n&iacute;\u017ee uveden&yacute;ch m\u011b\u0159en&iacute; typu CFFF:<\/p>\n<ul>\n<li>P\u0159ed ponorem, aby se stanovil u ka\u017ed&eacute;ho pot&aacute;p\u011b\u010de z&aacute;klad<\/li>\n<li>P\u0159i dosa\u017een&iacute; 30timetrov&eacute; hloubky<\/li>\n<li>P\u011bt minut p\u0159ed v&yacute;stupem k hladin\u011b<\/li>\n<li>Po v&yacute;stupu na hladinu<\/li>\n<li>T\u0159icet minut po ponoru<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Podle autor\u016f se jednalo o v\u016fbec prvn&iacute; m\u011b\u0159en&iacute; &uacute;\u010dink\u016f IGN u standardn&iacute; populace za odli&scaron;n&yacute;ch podm&iacute;nek prost\u0159ed&iacute; (tj. za sucha oproti mokru, s ochrann&yacute;m od\u011bvem \u010di bez takov&eacute; ochrany, bez zohled\u0148ov&aacute;n&iacute; rozd&iacute;lu mezi mo\u0159skou vodou a vodou v baz&eacute;nu). Zji&scaron;t\u011bn&eacute; v&yacute;sledky byly p\u0159ekvapiv&eacute;.<\/p>\n<p style=\"text-align: justify;\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" src=\"\/wp-content\/uploads\/imgimport\/1013\/graphiquecolle-3.jpg\" style=\"width: auto;\"><\/p>\n<p style=\"\"><\/p>\n<p style=\"text-align: justify;\">Jak ukazuje obr&aacute;zek 1, v&yacute;sledky CFFF byly pozoruhodn\u011b shodn&eacute; u v&scaron;ech t\u0159&iacute; prost\u0159ed&iacute;. Za prv&eacute;, hodnoty CFFF u pot&aacute;p\u011b\u010d\u016f se p\u0159i dosa\u017een&eacute; hloubce zv&yacute;&scaron;ily, co\u017e nazna\u010dovalo zv&yacute;&scaron;enou kognitivn&iacute; funkci. O 15 minut pozd\u011bji do&scaron;lo k v&yacute;znamn&eacute;mu poklesu hodnot CFFF, tedy ke kognitivn&iacute; degradaci, nebo\u0165 z\u0159ejm\u011b za\u010d&iacute;nala p\u016fsobit nark&oacute;za. Kupodivu toto zhor&scaron;en&iacute; p\u0159etrv&aacute;valo i na hladin\u011b a je&scaron;t\u011b 30 minut po ponoru.<\/p>\n<p style=\"text-align: justify;\">Toto p\u0159etrv&aacute;v&aacute;n&iacute; nark&oacute;zy nazna\u010duje, \u017ee star&aacute; rada jednodu&scaron;e vystoupat o n\u011bkolik metr\u016f v&yacute;&scaron;e p\u0159i n&aacute;stupu narkotick&eacute;ho stavu je z\u0159ejm\u011b naprosto ne&uacute;\u010dinn&aacute; strategie. T&iacute;m se potvrdila validita CFFF jako m\u011b\u0159ic&iacute;ho n&aacute;stroje za hyperbarick&yacute;ch podm&iacute;nek a bylo z\u0159ejm&eacute;, \u017ee v&yacute;sledky CFFF jsou srovnateln&eacute; s t\u011bmi p\u0159i pou\u017eit&iacute; PEBL.<\/p>\n<h3 style=\"\">Stla\u010den&yacute; vzduch pat\u0159&iacute; do pneumatik?<\/h3>\n<p style=\"text-align: justify;\">I kdy\u017e se p\u0159i ponorech projevoval podobn&yacute; pr\u016fb\u011bh, mezi ob\u011bma plyny existoval jeden v&yacute;znamn&yacute; rozd&iacute;l. EANx byl typick&yacute; pro v\u011bt&scaron;&iacute; mozkovou aktivitu, ne\u017e tomu bylo u ponor\u016f se vzduchem a tak&eacute; tam doch&aacute;zelo k m&iacute;rn\u011bj&scaron;&iacute;mu zhor&scaron;en&iacute; v pozd\u011bj&scaron;&iacute; f&aacute;zi ponoru a po ponoru.
