P\u0159i pr\u00e1ci na v\u00fdvoji n\u00e1stroje pro lep\u0161\u00ed kvantifikaci dekompresn\u00edho stresu u pot\u00e1p\u011b\u010d\u016f se p\u0159i \u0159e\u0161en\u00ed ned\u00e1vn\u00e9ho projektu poda\u0159ilo korelovat metriku historicky pou\u017e\u00edvanou v kardiovaskul\u00e1rn\u00ed medic\u00edn\u011b a zn\u00e1m\u00e9 markery dekompresn\u00edho stresu, p\u0159i\u010dem\u017e ob\u011b tyto skute\u010dnosti se zkombinovaly s p\u0159\u00edstupem, kter\u00fd se d\u00edv\u00e1 na dekompresn\u00ed onemocn\u011bn\u00ed nikoli jako na nemoc zp\u016fsobenou n\u011bkolika \u201c\u0161patn\u00fdmi\u201d bublinami, ale jako mnohostrannou nemoc vyvolanou zn\u00e1m\u00fdmi faktory a podporovanou fyziologick\u00fdmi reakcemi na tyto spou\u0161t\u011b\u010de.<\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"8957\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" style=\"width: auto;\" src=\"\/wp-content\/uploads\/imgimport\/1152\/diver-1500.jpg\" \/><\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"9660\">Neznamen\u00e1 to, \u017ee by dekompresn\u00ed nemoc u\u017e nebyla o bublin\u00e1ch, ale jde o to, \u017ee se nejedn\u00e1 jen o bubliny. V posledn\u00edm desetilet\u00ed \u0159ada nov\u00fdch v\u00fdzkum\u016f ilustrovala model dekompresn\u00ed nemoci, kter\u00fd se op\u00edr\u00e1 o neust\u00e1le rostouc\u00ed po\u010det fyziologick\u00fdch reakc\u00ed a st\u00e1le slo\u017eit\u011bj\u0161\u00ed biochemick\u00e9 mechanismy. Katalyz\u00e1torem tohoto posunu v ch\u00e1p\u00e1n\u00ed bylo p\u0159edev\u0161\u00edm zapojen\u00ed v\u00fdzkumu endoteli\u00e1ln\u00ed dysfunkce do dekompresn\u00ed nemoci. Procesy signalizace a odezvy z endoteli\u00e1ln\u00edch bun\u011bk byly spojeny s \u010detn\u00fdmi chorobn\u00fdmi procesy a z\u00e1jem o tyto procesy a pod\u00edl z\u00e1n\u011btliv\u00fdch proces\u016f na zn\u00e1m\u00fdch onemocn\u011bn\u00edch doslova explodoval. Endoteli\u00e1ln\u00ed dysfunkce je slo\u017eit\u00e1 a st\u00e1le se vyv\u00edj\u00ed, ale d\u016fle\u017eit\u00e9 je pochopit, \u017ee modern\u00ed v\u00fdzkum uk\u00e1zal, \u017ee i nemoci, o kter\u00fdch jsme si mysleli, \u017ee jsou ji\u017e dob\u0159e prostudovan\u00e9 a pochopen\u00e9, nemus\u00ed b\u00fdt produktem jedin\u00e9ho \u00fato\u010dn\u00e9ho \u010dinidla, n\u00fdbr\u017e kombinac\u00ed spou\u0161t\u011b\u010de a reakc\u00ed t\u011bla \u2013 v n\u011bkter\u00fdch p\u0159\u00edpadech m\u016f\u017ee b\u00fdt onemocn\u011bn\u00ed dokonce zcela zp\u016fsobeno reakc\u00ed t\u011bla na tento spou\u0161t\u011b\u010d.<\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"10633\">Bohu\u017eel to znamen\u00e1, \u017ee ji\u017e nem\u016f\u017eeme uva\u017eovat o dekompresn\u00ed nemoci jako o jednoduch\u00e9 nemoci zp\u016fsoben\u00e9 bublinami v krevn\u00edm \u0159e\u010di\u0161ti. M\u00edsto toho byla endoteli\u00e1ln\u00ed dysfunkce korelov\u00e1na s hyperbarickou expozic\u00ed a v\u00fdzkumn\u00edci se kv\u016fli t\u00e9to skute\u010dnosti museli za\u010d\u00edt d\u016fkladn\u011b zab\u00fdvat studiem z\u00e1n\u011btliv\u00fdch proces\u016f a toho, co se vlastn\u011b d\u011bje skute\u010dn\u00fdmi ponory, kter\u00e9 prov\u00e1d\u00edme a dekompresn\u00ed nemoc\u00ed, kter\u00e1 m\u016f\u017ee n\u00e1sledovat.<\/p>\n<h3 style=\"text-align: left;\" data-renderer-start-pos=\"11200\">Role variability srde\u010dn\u00ed frekvence (HRV)<\/h3>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"11242\">Variabilita srde\u010dn\u00ed frekvence (HRV) se pou\u017e\u00edv\u00e1 jako potenci\u00e1ln\u00ed zkratka pro m\u011b\u0159en\u00ed specifick\u00fdch z\u00e1n\u011btliv\u00fdch reakc\u00ed. Byla korelov\u00e1na s \u0159adou stav\u016f, a to <span data-renderer-mark=\"true\" data-text-custom-color=\"#4c9aff\"><a href=\"https:\/\/linkinghub.elsevier.com\/retrieve\/pii\/0735109789904087\" target=\"_blank\" rel=\"noopener\">od cukrovky a\u017e po kardiovaskul\u00e1rn\u00ed onemocn\u011bn\u00ed<\/a><\/span> a uk\u00e1zalo se, <a href=\"https:\/\/scholar.google.com\/scholar_lookup?title=Effect+of+shallow+and+deep+SCUBA+dives+on+heart+rate+variability%2E&amp;journal=Front%2E+Physiol%2E&amp;author=Noh+Y.&amp;author=Posada-Quintero+H.+F.&amp;author=Bai+Y.&amp;author=White+J.&amp;author=Florian+J.+P.&amp;author=Brink+P.+R.&amp;publication_year=2018&amp;volume=9&amp;issue=110\" target=\"_blank\" rel=\"noopener\">\u017ee <span data-renderer-mark=\"true\" data-text-custom-color=\"#4c9aff\">se m\u011bn\u00ed s pot\u00e1p\u011bn\u00edm<\/span><\/a>. A pr\u00e1v\u011b tady vstupuje tento <span data-renderer-mark=\"true\" data-text-custom-color=\"#4c9aff\"><a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fphys.2020.00743\/full\" target=\"_blank\" rel=\"noopener\">nov\u00fd v\u00fdzkum<\/a><\/span> do hry. Bylo prok\u00e1z\u00e1no, \u017ee mikro\u010d\u00e1stice v krvi koreluj\u00ed se z\u00e1n\u011btem, HRV se m\u011bn\u00ed s pot\u00e1p\u011bn\u00edm a koreluje se z\u00e1n\u011btem a mikro\u010d\u00e1stice se zvy\u0161uj\u00ed s vystaven\u00edm <span data-renderer-mark=\"true\" data-text-custom-color=\"#4c9aff\"><a href=\"https:\/\/doi.org\/10.1074\/jbc.M113.543702\" target=\"_blank\" rel=\"noopener\">inertn\u00edm hyperbarick\u00fdm plyn\u016fm<\/a><\/span>, tak\u017ee je mo\u017en\u00e9, \u017ee HRV by mohla b\u00fdt pou\u017eita jako vhodn\u00fd n\u00e1stroj pro m\u011b\u0159en\u00ed z\u00e1n\u011btliv\u00fdch reakc\u00ed u pot\u00e1p\u011b\u010d\u016f a potenci\u00e1ln\u00ed odhad rizika dekompresn\u00ed nemoci. Pokud to zn\u00ed ambici\u00f3zn\u011b, tak jen proto, \u017ee tomu tak skute\u010dn\u011b je, ale pokud by bylo mo\u017en\u00e9 potvrdit korelaci mezi variabilitou srde\u010dn\u00ed frekvence a fyziologick\u00fdm stresem vyvolan\u00fdm dekompres\u00ed, uk\u00e1zalo by se to jako nesm\u00edrn\u011b \u00fa\u010dinn\u00fd n\u00e1stroj pro pochopen\u00ed, jak a pro\u010d u pot\u00e1p\u011b\u010d\u016f doch\u00e1z\u00ed k onemocn\u011bn\u00ed a jak\u00e9 jsou fyziologick\u00e9 reakce na dekompresn\u00ed stres. T\u00edm se p\u0159\u00edmo nab\u00edz\u00ed metoda pro odhad dekompresn\u00edho rizika v ter\u00e9nu. Tak daleko sice je\u0161t\u011b nejsme, ale ur\u010dit\u011b se jedn\u00e1 o vzru\u0161uj\u00edc\u00ed prvn\u00ed krok na cest\u011b, o kter\u00e9 sn\u00ed hlavn\u011b \u201ebublinkov\u00ed maniaci\u201c.<\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"11242\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" style=\"width: auto;\" src=\"\/wp-content\/uploads\/imgimport\/1152\/diver2-1500.jpg\" \/><\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"12395\">Z\u00e1kladn\u00ed v\u00fdzkum je trochu slo\u017eit\u00fd, ale j\u00e1drem t\u00e9to studie je my\u0161lenka, \u017ee HRV lze korelovat se z\u00e1n\u011btliv\u00fdmi markery spojen\u00fdmi s dekompresn\u00ed nemoc\u00ed a d\u00e1 se p\u0159\u00edmo pou\u017e\u00edt jako m\u011b\u0159\u00edtko rizika dekompresn\u00ed nemoci. Konkr\u00e9tn\u011b se tento t\u00fdm pod\u00edval na markery odebran\u00e9 ze vzork\u016f krve a v\u00fdsledky EKG, aby za\u010dal korelovat HRV s t\u011bmito z\u00e1n\u011btliv\u00fdmi markery. 28 dobrovoln\u00edk\u016f pro\u0161lo dv\u011bma samostatn\u00fdmi profily ponor\u016f v jednom ze t\u0159\u00ed za\u0159\u00edzen\u00ed. <span data-renderer-mark=\"true\" data-text-custom-color=\"#36b37e\">V\u0161echny profily ponor\u016f m\u011bly stejnou celkovou dobu dekomprese a p\u0159esycen\u00ed plynem, i kdy\u017e se li\u0161ily v hloubk\u00e1ch dekompresn\u00edch zast\u00e1vek, aby obsahovaly i ur\u010dit\u00e9 odchylky. Intervaly mezi ponory a intervaly sb\u011bru dat byly stejn\u00e9.<\/span><\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"13048\"><span data-renderer-mark=\"true\" data-text-custom-color=\"#36b37e\">V\u00fdzkumn\u00edci srovnali data mezi dv\u011bma profily, aby je bylo mo\u017en\u00e9 analyzovat jako jeden soubor dat.<\/span> V z\u00e1vislosti na za\u0159\u00edzen\u00ed \u00fa\u010dastn\u00edci pou\u017e\u00edvali bu\u010f rebreathery s uzav\u0159en\u00fdm okruhem, d\u00fdchac\u00ed plyn dod\u00e1van\u00fd hyperbarickou komorou nebo pot\u00e1p\u011b\u010dsk\u00e9 vybaven\u00ed s otev\u0159en\u00fdm okruhem. P\u0159ed ponorem a po ponoru byly odebr\u00e1ny vzorky EKG a krve s 30minutov\u00fdm vyrovn\u00e1vac\u00edm intervalem a po ka\u017ed\u00e9m ponoru a mezi ponory byl vy\u017eadov\u00e1n minim\u00e1ln\u011b 48hodinov\u00fd interval, aby se zabr\u00e1nilo zkreslen\u00ed dat v d\u016fsledku p\u0159etrv\u00e1vaj\u00edc\u00edch \u00fa\u010dink\u016f p\u0159edchoz\u00ed expozice.<\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"13048\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" style=\"width: auto;\" src=\"\/wp-content\/uploads\/imgimport\/1152\/image1-1500.jpg\" \/><\/p>\n<p style=\"text-align: center;\" data-renderer-start-pos=\"13048\">Obr\u00e1zek: V\u00fdvojov\u00fd diagram postupu \u2013 <em data-renderer-mark=\"true\">asociace mezi variabilitou srde\u010dn\u00ed frekvence a dekompres\u00ed \u2013 indukovan\u00fd fyziologick\u00fd stres<\/em><\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"13713\">V\u00fdznamn\u00e9 z\u00edskan\u00e9 v\u00fdsledky byly analyzov\u00e1ny zp\u016fsobem, kter\u00fd nejl\u00e9pe popsali sami <a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fphys.2020.00743\/full\" target=\"_blank\" rel=\"noopener\">auto\u0159i<\/a>, a pot\u00e9 byly pou\u017eity k vytvo\u0159en\u00ed teoretick\u00e9ho modelu pro extrapolaci v\u00fdsledk\u016f z\u00edskan\u00fdch z relativn\u011b mal\u00e9 skupiny vzork\u016f a jejich modelov\u00e1n\u00ed na datov\u00e9m souboru zahrnuj\u00edc\u00edm p\u0159ibli\u017en\u011b 1000 ponor\u016f. <span data-renderer-mark=\"true\" data-text-custom-color=\"#00b8d9\">Mezi postanalytick\u00fdmi datov\u00fdmi body existuje \u0159ada zaj\u00edmav\u00fdch sm\u011br\u016f a cest, ale zam\u011b\u0159en\u00ed na vztah mezi HRV a z\u00e1n\u011btliv\u00fdmi markery uk\u00e1zalo statisticky v\u00fdznamnou zm\u011bnu, kter\u00e1 by mohla b\u00fdt skute\u010dn\u00fdm p\u0159\u00edslibem. Z\u00edskan\u00e1 data mohou b\u00fdt sice hroziv\u00e1, ale kl\u00ed\u010dov\u00e9 trendy lze vid\u011bt ve vztahu mezi HF a SDNN, Annexinem a MP a pozitivn\u00ed asociaci mezi LF a CD66b+ a CD31+ MP. Pro ty z v\u00e1s, kte\u0159\u00ed jste se ztratili v t\u00e9 z\u00e1plav\u011b zkratek, to v\u0161e v podstat\u011b znamen\u00e1, \u017ee se zd\u00e1, \u017ee v\u00edce korelac\u00ed dat se sbli\u017euje zp\u016fsobem, kter\u00fd poskytuje statisticky platn\u00fd d\u016fkaz pro hypot\u00e9zy v\u00fdzkumn\u00edk\u016f.<\/span><\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"13713\"><img decoding=\"async\" class=\"fr-dib fr-draggable\" style=\"width: auto;\" src=\"\/wp-content\/uploads\/imgimport\/1152\/image2-1500.jpg\" \/><\/p>\n<p style=\"text-align: center;\" data-renderer-start-pos=\"13713\">N\u00e1sledn\u00e9 vztahy mezi ukazateli prokazuj\u00edc\u00edmi HRV, MPO (tj. exym myeloperoxid\u00e1zu) a MP (mikro\u010d\u00e1stice) &#8211; <em data-renderer-mark=\"true\">asociace mezi variabilitou srde\u010dn\u00ed frekvence a dekompres\u00ed \u2013 vyvolan\u00fd fyziologick\u00fd stres<\/em><\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"14758\"><em data-renderer-mark=\"true\">Pozn\u00e1mka: LF a HF jsou n\u00edzkofrekven\u010dn\u00ed a vysokofrekven\u010dn\u00ed filtry pou\u017e\u00edvan\u00e9 b\u011bhem pou\u017e\u00edv\u00e1n\u00ed EKG \u2013 oba p\u0159edstavuj\u00ed r\u016fzn\u00e9 aspekty elektrokardiografick\u00e9 anal\u00fdzy HRV. Rozd\u00edly nejsou z\u00e1sadn\u00ed pro celkov\u00e9 pochopen\u00ed tohoto v\u00fdzkumu, ale <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5624990\/\" target=\"_blank\" rel=\"noopener\">z\u00e1jemc\u016fm je k dispozici v\u00edce informac\u00ed<\/a> .<\/em><\/p>\n<h3 style=\"text-align: left;\" data-renderer-start-pos=\"15027\">Z\u00e1v\u011br<\/h3>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"15034\">Pokud v\u00e1m tohle v\u0161echno p\u0159ipad\u00e1 na b\u011b\u017en\u00e9 odpoledn\u00ed \u010dten\u00ed a\u017e trochu moc n\u00e1ro\u010dn\u00e9, je tomu tak proto, \u017ee to slo\u017eit\u00e9 opravdu je. Po\u010det prom\u011bnn\u00fdch, kter\u00e9 ovliv\u0148uj\u00ed z\u00e1n\u011btliv\u00e9 procesy, dekompresn\u00ed onemocn\u011bn\u00ed a dokonce i ka\u017edodenn\u00ed \u00fadaje o HRV u stejn\u00e9ho subjektu, je t\u00e9m\u011b\u0159 nezm\u011b\u0159iteln\u00fd a zohledn\u011bn\u00ed co nejv\u011bt\u0161\u00edho po\u010dtu vy\u017eaduje obrovsk\u00e9 mno\u017estv\u00ed z\u00e1kladn\u00edho v\u00fdzkumu a experiment\u00e1ln\u00edho pl\u00e1nov\u00e1n\u00ed. Nutn\u00e9 je pochopit modely natolik, aby bylo mo\u017en\u00e9 potvrdit korektnost v\u00fdzkumu, a\u017e pot\u00e9 je mo\u017en\u00e9 se zam\u011b\u0159it na v\u00fdsledky \u2013 kter\u00e9 v tomto p\u0159\u00edpad\u011b prim\u00e1rn\u011b nazna\u010duj\u00ed korelaci mezi HRV po dekompresi a ukazateli fyziologick\u00e9ho z\u00e1n\u011btu a stresu, na co\u017e upozor\u0148uj\u00ed v\u00fdzkumn\u00ed pracovn\u00edci. V \u017e\u00e1dn\u00e9m p\u0159\u00edpad\u011b to nen\u00ed d\u016fvod, pro\u010d si po\u0159\u00eddit m\u011b\u0159i\u010d srde\u010dn\u00edho tepu a zkr\u00e1tit si dekompresi co mo\u017en\u00e1 nejv\u00edce, ale jedn\u00e1 se o slibn\u00fd v\u00fdsledek a vzru\u0161uj\u00edc\u00ed potenci\u00e1l do budoucna. Za zm\u00ednku stoj\u00ed, \u017ee zv\u00ed\u0159ec\u00ed modely jsou v rozporu s \u00fadaji tohoto v\u00fdzkumu a v\u00fdsledky se tak\u00e9 mohou li\u0161it v jednotliv\u00fdch p\u0159\u00edpadech subjekt\u016f trp\u00edc\u00edch dekompresn\u00ed nemoc\u00ed, nicm\u00e9n\u011b mo\u017enosti budouc\u00edho v\u00fdzkumu jsou obrovsk\u00e9.<\/p>\n<p style=\"text-align: justify;\" data-renderer-start-pos=\"16095\">Pokud lze tento vztah mezi HRV a dekompresn\u00edm stresem d\u00e1le ov\u011b\u0159it, je mo\u017en\u00e9, \u017ee by dekompresn\u00ed modely mohli \u00a0ov\u011b\u0159it pot\u00e1p\u011b\u010di kvalitativn\u011b v re\u00e1ln\u00e9m \u010dase a kone\u010dn\u011b by bylo mo\u017en\u00e9 skute\u010dn\u011b porozum\u011bt nezn\u00e1m\u00fdm skute\u010dnostem souvisej\u00edc\u00edm s p\u0159esycen\u00edm tk\u00e1n\u00ed a dekompresn\u00ed nemoc\u00ed. Jestli\u017ee se tento model skute\u010dn\u011b ov\u011b\u0159\u00ed, mohli bychom se st\u00e1t sv\u011bdky z\u00e1sadn\u00ed zm\u011bny ve zp\u016fsobu, jak\u00fdm je t\u0159eba pl\u00e1novat dekompresi a jak se m\u00e1 dekompresn\u00ed nemoc ch\u00e1pat a l\u00e9\u010dit. D\u00edky schopnosti popul\u00e1rn\u00ed spot\u0159ebitelsk\u00e9 technologie identifikovat srde\u010dn\u00ed arytmie pomoc\u00ed n\u00e1ramkov\u00fdch hodinek nen\u00ed nemo\u017en\u00e9, \u017ee bychom mohli v budoucnu vid\u011bt modelov\u00e1n\u00ed HRV za\u010dlen\u011bn\u00e9 do pot\u00e1p\u011b\u010dsk\u00fdch po\u010d\u00edta\u010d\u016f.
