Une oxyg\u00e9noth\u00e9rapie hyperbare peut \u00eatre administr\u00e9e dans un caisson monoplace (destin\u00e9 \u00e0 une seule personne) ou multiplace (pouvant accueillir deux patients ou plus et permettant l&rsquo;acc\u00e8s d&rsquo;assistants \u00e0 tout moment pendant un traitement).<br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">Les effets de l&rsquo;oxyg\u00e8ne hyperbare sont principalement attribu\u00e9s \u00e0 l&rsquo;augmentation de la tension d&rsquo;oxyg\u00e8ne et du taux d&rsquo;oxyg\u00e8ne sanguin, qui \u00e0 leur tour affectent les tissus de l&rsquo;organisme. L&rsquo;oxyg\u00e8ne hyperbare est utilis\u00e9 pour soigner de nombreuses maladies. Dans des conditions atmosph\u00e9riques normales, la majeure partie de l&rsquo;oxyg\u00e8ne transport\u00e9 par le sang est li\u00e9e \u00e0 l&rsquo;h\u00e9moglobine ; seule une petite quantit\u00e9 d&rsquo;oxyg\u00e8ne se dissout dans les composants liquides du sang. Lors de l&rsquo;augmentation de la pression au sein d&rsquo;un caisson hyperbare dans lequel un patient respire de l&rsquo;oxyg\u00e8ne pur, la pression partielle de l&rsquo;oxyg\u00e8ne inhal\u00e9 cro\u00eet proportionnellement. Pour chaque atmosph\u00e8re (10 m\u00e8tres d&rsquo;eau de mer) suppl\u00e9mentaire par rapport \u00e0 la pression atmosph\u00e9rique au niveau de la mer, une atmosph\u00e8re d&rsquo;oxyg\u00e8ne suppl\u00e9mentaire (soit l&rsquo;\u00e9quivalent de 200 % d&rsquo;oxyg\u00e8ne) est inspir\u00e9e.<br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">Outre les effets b\u00e9n\u00e9fiques de l&rsquo;oxyg\u00e8ne, la pression hydrostatique offre \u00e9galement l&rsquo;avantage de comprimer les bulles d&rsquo;azote, r\u00e9duisant ainsi leur volume et att\u00e9nuant les sympt\u00f4mes. Dans quels cas administre-t-on de l&rsquo;oxyg\u00e8ne hyperbare? L&rsquo;oxyg\u00e9noth\u00e9rapie hyperbare est utilis\u00e9e comme traitement primaire ou d&rsquo;appoint pour une vari\u00e9t\u00e9 de conditions m\u00e9dicales et de l\u00e9sions. Cette th\u00e9rapie a \u00e9t\u00e9 approuv\u00e9e par l&rsquo;UHMS (Undersea and Hyperbaric Medical Society) aux \u00c9tats-Unis pour les 13 indications suivantes. Pour plus d&rsquo;informations, consultez \u00e9galement le site Web <a href=\"http:\/\/www.echm.org\">www.echm.org<\/a>).<\/p>\n<ul>\n<li>L&rsquo;embolie gazeuse art\u00e9rielle (AGE)<\/li>\n<li>L&rsquo;intoxication au monoxyde de carbone (CO) et l&rsquo;intoxication au CO compliqu\u00e9e par une intoxication au cyanure;<\/li>\n<li>La myosite clostridiale et la gangr\u00e8ne gazeuse;<\/li>\n<li>Les l\u00e9sions par \u00e9crasement, les syndromes de loge et autres isch\u00e9mies traumatiques aigu\u00ebs;<\/li>\n<li>La maladie de d\u00e9compression;<\/li>\n<li>L&rsquo;aide \u00e0 la cicatrisation de certaines l\u00e9sions probl\u00e9matiques;<\/li>\n<li>Les pertes sanguines exceptionnelles (an\u00e9mie);<\/li>\n<li>Les abc\u00e8s intracr\u00e2niens;<\/li>\n<li>Les infections n\u00e9crosantes des tissus mous;<\/li>\n<li>L&rsquo;ang\u00e9ite n\u00e9crosante (r\u00e9fractaire);<\/li>\n<li>Le syndrome d&rsquo;irradiation diff\u00e9r\u00e9 (n\u00e9crose des os et des tissus mous);<\/li>\n<li>Les greffes et lambeaux cutan\u00e9s (compromis);<\/li>\n<li>Les br\u00fblures thermiques.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Quel est le mode d&rsquo;administration de l&rsquo;oxyg\u00e9noth\u00e9rapie hyperbare ?<\/strong><br \/>Les th\u00e9rapies par oxyg\u00e9noth\u00e9rapie hyperbare peuvent comprendre une seule recompression pour des affections aigu\u00ebs, et jusqu&rsquo;\u00e0 20 ou 40 traitements, voire plus pour des affections m\u00e9dicales chroniques. Chaque traitement dure g\u00e9n\u00e9ralement une \u00e0 deux heures. Le nombre de traitements d\u00e9pend de l&rsquo;\u00e9volution du patient et de ses sympt\u00f4mes. Pour chaque traitement, la pression du caisson et la dur\u00e9e du traitement sont d\u00e9termin\u00e9es en fonction du diagnostic et des proc\u00e9dures de traitement sp\u00e9cifiques \u00e0 l&rsquo;infrastructure. Avant d&rsquo;entrer dans le caisson hyperbare, les patients et le personnel traitant doivent porter des v\u00eatements d&rsquo;h\u00f4pital sp\u00e9ciaux. Aucun mat\u00e9riau inflammable \u00e0 base de p\u00e9trole, y compris les habits des patients ou autres produits pouvant produire des \u00e9tincelles, n&rsquo;est admis au sein du caisson.<\/p>\n<p style=\"text-align: justify;\"><strong>Consid\u00e9rations relatives au traitement<\/strong><br \/>\u00c9tant donn\u00e9 que l&rsquo;oxyg\u00e9nation hyperbare induit la contraction et l&rsquo;expansion de l&rsquo;air se trouvant dans les cavit\u00e9s a\u00e9riennes de l&rsquo;organisme, toute maladie ou tout trouble pouvant interf\u00e9rer avec ce ph\u00e9nom\u00e8ne requiert une attention particuli\u00e8re. Toute condition m\u00e9dicale pouvant alt\u00e9rer l&rsquo;oxyg\u00e9nation du sang ou la circulation sanguine dans les tissus de l&rsquo;organisme r\u00e9duira l&rsquo;efficacit\u00e9 de l&rsquo;oxyg\u00e9nation hyperbare.<\/p>\n<p style=\"text-align: justify;\"><strong>Un dysfonctionnement du tube d&rsquo;Eustache<\/strong> (probl\u00e8mes de compensation) ou encore une maladie pulmonaire bulleuse sont des exemples de conditions m\u00e9dicales d\u00e9favorables. Un trouble du conduit auditif emp\u00eache l&rsquo;\u00e9quilibrage de l&rsquo;oreille moyenne par rapport aux pressions ambiantes. Ce type de trouble requiert parfois la perforation temporaire du tympan (tympanostomie ou myringotomie, perforation ou introduction d&rsquo;une sonde dans la membrane tympanique respectivement) pr\u00e9alablement au traitement. Les maladies pulmonaires bulleuses (pr\u00e9sence de bulles d&rsquo;air dans les poumons) sont \u00e9galement consid\u00e9r\u00e9es comme une relative contre-indication \u00e0 l&rsquo;oxyg\u00e9noth\u00e9rapie hyperbare, car qu&rsquo;elles pourraient pr\u00e9disposer le patient au pneumothorax ou \u00e0 l&rsquo;embolie gazeuse art\u00e9rielle.<br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">En outre, l&rsquo;oxyg\u00e9noth\u00e9rapie hyperbare n&rsquo;est pas indiqu\u00e9e pour les femmes enceintes, sauf dans le cas d&rsquo;une maladie aigu\u00eb pour laquelle un report du traitement comporterait un risque plus \u00e9lev\u00e9 qu&rsquo;une exposition du foetus au traitement hyperbare. Les Drs Reza Gorji et Enrico Camporesi ont constat\u00e9 que lors d&rsquo;une grossesse, il se produit une redistribution des liquides de l&rsquo;organisme dans les tissus p\u00e9riph\u00e9riques, \u00e0 l&rsquo;\u00e9cart de la circulation centrale. Ce ph\u00e9nom\u00e8ne pourrait pr\u00e9disposer les femmes enceintes \u00e0 une r\u00e9tention d&rsquo;azote et augmenter d\u00e8s lors le risque de MDD.<br \/>Les effets de l&rsquo;absorption d&rsquo;azote et les risques de MDD chez le foetus sont moins clairs. Les poumons foetaux ne participent pas aux \u00e9changes gazeux et sont donc incapables de filtrer les microbulles pouvant \u00eatre pr\u00e9sentes dans leur circulation. Ces bulles peuvent \u00eatre transmises par la m\u00e8re via le placenta ou peuvent se former spontan\u00e9ment chez le foetus.<br \/>&nbsp;<\/p>\n<p style=\"text-align: justify;\">Comme dans le cas d&rsquo;une MDD, la formation de bulles chez le foetus peut avoir des effets n\u00e9fastes. Celles-ci peuvent entraver le d\u00e9veloppement et la fonction des organes et provoquer des malformations cong\u00e9nitales, voire un avortement spontan\u00e9. Par ailleurs, la plong\u00e9e peut modifier la physiologie d&rsquo;autres substances dans l&rsquo;organisme.
